In a radiology results report:

CONCLUSION
Pleuropneumopathie basale droite au décours.

My first impression is that it means that the illness is serious/in an end stage, but I am not sure if that's correct and if so, I am not sure how to translate it. I would appreciate any help!

Au décours in medical terms usually means following (following the treatment, following the operation ect.)

Based on the x-ray findings you quoted in your other query (https://www.proz.com/kudoz/french_to_english/medical_cardiol... ), it is probably the end of the disease rather than the end of the patient ;-)
"Resolving pneumonia" is not really a standard expression in English reports, I hope some English colleagues will step in and provide a natural English expression.
