The purpose of this study was to determine if prophylactic rofecoxib would significantly reduce postendodontic pain, when compared ibuprofen or placebo. An additional objective was to establish if any relationship exists between periapical diagnosis and the need for additional medication after completion of pulpectomy. A total of 45 patients consented to a double-blind, single-dose oral administration of 50 mg of rofecoxib, 600 mg of ibuprofen, or a placebo before conventional root canalar therapy.
The root canalar treatment was performed in two appointments. Patient-reported visual analog scale ratings of pain intensity were conducted upon initial clinical presentation and at 4, 8, 12, 24, 48 and 72 h after completion of pulpectomy. Results showed that at the 4- and 8-h periods, both rofecoxib and ibuprofen provided significantly better pain relief than placebo. Patients with a periapical diagnosis of acute apical periodontitis showed a significantly increased need for additional medication after completion of pulpectomy compared with all other periapical diagnoses.

L’obiettivo di questo studio era di stabilire se il rofecoxib profilattico
avrebbe ridotto significativamente il dolore, messo a confronto con l’ibuprofene o il placebo. Un ulteriore scopo era quello di trovare le relazioni esistenti tra la diagnosi periapicale e il bisogno di medicazioni aggiuntive dopo il completamento della pulpectomia. Un totale di 45 pazienti ha acconsentito ad effettuare un test a doppio cieco, con la somministrazione orale di una singola dose di 50 mg di rofecoxib, 600 mg di ibuprofene o di un placebo prima della convenzionale terapia canalare. Quest’ultima è stata effettuata in 2 sedute. Le scale di valutazione visive e analogiche (VAS) dell’intensità del dolore riferite dal paziente, sono state condotte al momento della presentazione clinica e a 4, 8, 12, 24, 48 e 72 h dopo il completamento della pulpectomia. I risultati hanno mostrato che dopo 4 e 8 h sia il rofecoxib che l’ ibuprofene hanno portato ad una attenuazione molto maggiore rispetto al placebo. I pazienti con una diagnosi di periodontite apicale acuta hanno richiesto molto di più una medicazione aggiuntiva dopo la pulpectomia rispetto a coloro che presentavano una diagnosi periapicale.
