Texte source - français 1. EPIDEMIOLOGIE
L'adénocarcinome prostatique est rare avant 50 ans, puis devient plus fréquent pour atteindre un pic de fréquence au cours de la 8e décennie.
En France il y a 40000 nouveaux cas de cancers de prostate par an (en 2000) et 9000 décès dus à cette maladie par an : c’est devenu le cancer le plus fréquent (devant le cancer du poumon).
Ce phénomène va s'amplifier puisque la population de plus de 65 ans augmente de 20 à 25 % tous les 10 ans.
Néanmoins, la mortalité augmente moins vite que l'incidence ce qui suggère que l'augmentation de l'incidence est plus liée aux progrès diagnostiques qu'à une réelle augmentation de la maladie.
La fréquence des cancers de prostate autopsiques, pour chaque tranche d'âge, est la suivante : 30 % entre 50 et 59 ans, 40 % entre 60 et 79 ans, 67 % entre 80 et 89 ans. On estime que 90 % des adénocarcinomes prostatiques survenant chez l'homme de plus de 70 ans ne sont pas diagnostiqués.
La mortalité du cancer de la prostate est importante puisqu'elle représente la troisième cause de mortalité par cancer chez l'homme après les cancers bronchiques et colorectaux.
Le cancer de la prostate est plus fréquent en Occident. L'incidence la plus élevée se trouve chez les Noirs américains (100,2 pour 100 000 habitants par an) et la plus basse chez les Chinois de Shanghai (0,8 pour 100 000 hommes par an). Mais il est difficile de faire la part entre un éventuel facteur génétique et les éléments de l'environnement. Il existe une nette augmentation de fréquence du cancer de la prostate chez les parents de patients atteints de cancer de prostate, ce qui est en faveur d'une influence du génome dans le cancer de la prostate.
De nombreux faits cliniques expérimentaux sont en faveur d'une origine "hormonale" du cancer de la prostate : androgéno-dépendance de la plupart des cancers de prostate, absence de cancer de la prostate chez l'eunuque, induction possible du cancer prostatique chez certaines souches de rats par administration chronique d'oestrogènes et d'androgènes.
2. ANATOMIE
La prostate est une glande située sous la vessie et autour de l’urètre et est constituée de deux portions, centrale et périphérique. Le cancer de la prostate prend le plus souvent naissance dans la portion périphérique de la prostate.
3. ANATOMIE PATHOLOGIQUE
Le cancer de la prostate est presque toujours un adénocarcinome, c’est-à-dire une tumeur développée aux dépens du tissu glandulaire prostatique.
L'adénocarcinome prostatique est caractérisé par son polymorphisme, ce qui rend difficile l'établissement d'un pronostic en fonction des données des biopsies prostatiques : il faut savoir que celles-ci sous-estiment souvent l’extension réelle du cancer et son degré de différenciation.
Macroscopiquement : il s'agit habituellement d'un nodule dur ou ferme, unique ou multiple, à contours irréguliers, généralement situé dans la partie périphérique de la prostate, accessible au toucher rectal.
A l'examen histologique, l'adénocarcinome prostatique est plus ou moins ressemblant au tissu prostatique sain. La tumeur est dite différenciée si la ressemblance est assez bonne, et peu
différenciée si cette ressemblance est presque inexistante. Dans une même tumeur, il est fréquent d'observer la coexistence d'architectures différentes.

Prostate adenocarcinoma is rare before the age of 50 and then becomes more frequent, reaching a peak between 70 and 80 years of age.
In France, there are 40,000 new cases of prostate cancer per year and 9,000 deaths are caused by this disease per year. It has become the most common form of cancer, ahead of lung cancer.
This trend is going increase as the population of over 65s rises by 20-25% every ten years.
Nevertheless, the mortality rate is rising more slowly than the incidence rate which suggests that the increase in incidence rates is linked more to progress in diagnosis than an actual increase in the disease.
The incidence of prostate cancer found at autopsy for each age bracket is as follows: 30% in 50-59 year olds, 40% in 60-79 year olds and 67% in 80-89 year olds. It is estimated that 90% of cases of prostatic adenocarcinoma in males ages over 70 are undiagnosed.
The prostate cancer mortality rate is significant and this form of cancer is the third leading cause of cancer death in men after bronchial and colorectal cancer.
Prostate cancer is most common in the West. The incidence rate is highest in Black American males (100.2 per 100,000 per year) and lowest in Chinese males from Shanghai (0.8 per 100,000 per year). However, it is difficult to differentiate between possible genetic factors and environmental factors. There is a clear increase in prostate cancer rates in the relatives of patients with prostate cancer which indicates that genetics may have an influence on prostate cancer.
Numerous clinical studies have found a relation between hormones and prostate cancer with the majority of prostate cancers being androgen-dependent, an absence of prostate cancer being found in eunuchs and chronic administration of oestrogens and androgens leading to a possible induction of prostate cancer in certain strains of rats.
2. Anatomy
The prostate is a gland situated under the bladder, encompasses the urethra and consists of 2 zones: the central zone and the peripheral zone. Prostate cancer most often originates in the peripheral zone of the prostate.
3. Pathological Anatomy
Prostate cancer is almost always an adenocarcinoma, i.e. a tumour which develops to the detriment of the prostatic glandular tissue.
Prostatic adenocarcinoma is characterised by its polymorphism which makes it difficult to establish a prognosis from the results of prostatic biopsies. It is important to know that these results often underestimate the real extent of the cancer and its degree of differentiation.
Macroscopic appearance: one or multiple hard/solid nodules with irregular boundaries, generally situated in the peripheral zone of the prostate and palpable on digital rectal examination.
Histological examination shows that prostatic adenocarcinoma resembles healthy prostatic tissue to a greater or lesser extent. The tumour is said to be well-differentiated if it closely resembles healthy tissue and undifferentiated if resemblance to the original tissue is almost in-existent. It is common for different cell structures to coexist in the same tumour.

4.1. Indications thérapeutiques
• Leucémies myéloïdes chroniques.
• Polyglobulie primitive.
• Thrombocytémie essentielle.
• Splénomégalie myéloïde, myélofibroses.
4.2. Posologie et mode d'administration
Ce médicament est administré en 1 ou 2, voire 3 prises par jour, en fonction de la dose quotidienne totale.
La dose administrée est basée sur le poids réel ou le poids idéal du patient, en prenant en considération le poids le plus faible.
Leucémie myéloïde chronique:
• traitement d'attaque: 30 à 50 mg/kg/24 heures.
• traitement d'entretien: 15 à 30 mg/kg/24 heures.
Splénomégalie myéloïde:
• 5 à 15 mg/kg/24 heures.
Thrombocytémie essentielle:
• dose d'attaque: 15 à 30 mg/kg/24 heures,
• dose d'entretien: 10 à 20 mg/kg/24 heures.
Polyglobulie primitive:
• dose d'attaque: 15 à 20 mg/kg/24 heures,
• dose d'entretien: 10 mg/kg/24 heures.
Il est important d'assurer, surtout en début de traitement, une diurèse abondante (voir rubrique 4.4. précautions d'emploi).
Si le patient n'est pas en mesure d'avaler la gélule, le contenu de la gélule peut être dispersé dans un verre d'eau et ingéré immédiatement. (voir rubrique 6.6).
Il est normal que certains excipients non solubles persistent en surface.
Insuffisance rénale:
HYDREA étant éliminé essentiellement par voie rénale, une diminution de la posologie devra être envisagée chez les patients présentant une insuffisance rénale. Une surveillance régulière des paramètres hématologiques est conseillée.
Insuffisance hépatique:
Les données disponibles sont insuffisantes pour recommander des ajustements posologiques chez les patients présentant une insuffisance hépatique. Une surveillance régulière des paramètres hématologiques est conseillée.
Enfants:
Dans l'état actuel des connaissances, il n'est pas possible de recommander une posologie particulière pour l'utilisation pédiatrique de HYDREA.
4.3. Contre-indications
HYDREA est contre-indiqué chez les patients ayant déjà présenté une réaction d'hypersensibilité à l'hydroxycarbamide ou à l'un des excipients.
HYDREA est contre-indiqué en association avec le vaccin antiamarile (fièvre jaune) (voir rubrique 4.5).
Grossesse et allaitement:
Son administration est formellement contre indiquée chez la femme enceinte et chez la femme en période d'allaitement (voir rubrique 4.6).
4.4. Mises en garde spéciales et précautions d'emploi
Mises en garde spéciales
Le traitement ne doit pas être instauré chez les patients ayant une fonction médullaire altérée.
Du fait de l'activité myélosuppressive de HYDREA une granulopénie, une thrombopénie et une anémie peuvent survenir (la granulopénie est la première et la plus fréquente de ces manifestations); ceci justifie une surveillance régulière des paramètres hématologiques.
La constatation d'une granulopénie inférieure à 1.000/mm3 ou d'une thrombopénie inférieure à 100.000/mm3 indique en principe l'interruption du traitement.
La récupération médullaire est rapide après l'arrêt du traitement (voir 4.4 Précautions d'emploi).
En cas d'anémie préexistante, il convient de la corriger avant d'initier le traitement.
Le risque de dépression médullaire étant plus élevé chez les patients ayant reçu une radiothérapie ou une chimiothérapie antérieure, l'administration d'HYDREA sera prudente chez ces patients.
Anomalies érythrocytaires: il est fréquent d'observer une macrocytose en début de traitement, d'évolution spontanément résolutive. Elle n'est pas liée à un déficit en acide folique ou en vitamine B12. La macrocytose pouvant masquer l'apparition secondaire d'une défaillance en acide folique, une administration prophylactique d'acide folique peut être justifiée.
Les patients ayant reçu une radiothérapie antérieure peuvent présenter une exacerbation d'un érythème radique lors de l'administration d'HYDREA.
Des lésions à type de vascularite cutanée incluant des ulcères et une gangrène sont survenues chez des patients présentant un syndrome myéloprolifératif lors de traitement par HYDREA. Ces lésions ont été le plus souvent rapportées chez des patients ayant reçu ou recevant un traitement par interféron. Du fait de leur évolution potentiellement grave chez ces patients le traitement par HYDREA doit être interrompu et une alternative thérapeutique doit être envisagée si nécessaire.
Surveillance de la diurèse: en cas de forte leucocytose, de splénomégalie notable ou d'uricémie élevée, conseiller des boissons abondantes afin d'obtenir une diurèse abondante.
Ce médicament est déconseillé avec les vaccins vivants atténués, la phénytoïne et la fosphénytoïne (voir rubrique 4.5).

4. Clinical Particulars
4.1. Therapeutic indications

4.2. Posology and method of administration
This medication is administered 1 to 2 (on occasion 3) times a day depending on the total daily dose.
The administered dose is based on the actual weight or the ideal weight of the patient, taking the lighter of the 2 weights.
Chronic myeloid leukemia:
• initial therapy: 30 to 50 mg/kg/24 hrs.
• maintenance therapy: 15 to 30 mg/kg/24 hrs.
Myelofibrosis:
• 5 to 15 mg/kg/24 hrs.
Essential Thrombocythaemia.
• initial therapy: 15 to 30 mg/kg/24 hrs,
• maintenance therapy: 10 to 20 mg/kg/24 hrs.
Polycythaemia vera:
• initial therapy: 15 to 20 mg/kg/24 hrs,
• maintenance therapy: 10 mg/kg/24 hrs.
It is important to ensure that there is abundant diuresis, particularly at the beginning of therapy ( see section 4.4, precaution for use ).
If the patient is unable to swallow the capsule, the contents of the capsule may be dissolved in a glass of water and ingested immediately (see section 6.6).
It is normal for some non-soluble excipients to remain on the surface of the water.
Renal failure:
As HYDREA is primarily excreted renally, dosage may have to be reduced in patients with renal failure. Regular hematological monitoring is advised.
Liver failure:
There is insufficient evidence available to recommend a change in posology for patients with liver failure. Regular hematological monitoring is advised.
Children:
It is not possible to recommend a paediatric posology for the use of HYDREA with the current state of knowledge.
4.3. Contraindications
HYDREA is contra-indicated in patients who have shown hypersensitivity to hydroxycarbamide or to one of its excipients.
HYDREA is contra-indicated in combination with the yellow fever vaccine (see section 4.5).
Pregnancy and breastfeeding:
HYDREA is absolutely contra-indicated in pregnant and breastfeeding women (see section 4.6).
4.4. Special warnings and precautions for use
Special warnings
The therapy should not be given to patients with impaired bone marrow function.
As the myelosuppressive effects of HYDREA may lead to granulocytopenia, thrombocytopenia and anemia, regular monitoring of hematological parameters is required. Granulocytopenia is the first and most frequent manifestation.
A granulocytopenia reading less than 1,000/mm3 or a thrombocytopenia reading less than 100,000/mm3 indicates the need to interrupt the treatment.
Marrow function recuperation is rapid once therapy has stopped (see 4.4 Precautions for use).
In the case of preexisting anemia, it is advised that this be resolved before starting therapy.
As the risk of myelosuppression is greater in patients receiving radiotherapy and chemotherapy, HYDREA should be administered with caution to these patients.
Red blood cell irregularities: macrocytosis is frequently observed at the beginning of therapy and is self-limiting over the course of the therapy. It is not linked to vitamin B12 or folic acid deficiency. As macrocytosis could mask the secondary appearance of folic acid deficiency, a prophylactic folic acid dose may be necessary.
Patients who have previously received radiation therapy may have an exacerbation of erythema while HYDREA is administered.
Patients with myeloproliferative neoplasm may present cutaneous vasculitis lesions (including ulcers) and gangrene during HYDREA treatment. These lesions have been reported most frequently by patients receiving interferon treatment. Due to their potentially severe clinical outcomes in such patients, HYDREA treatment must be discontinued and, if necessary, an alternative therapy must be considered.
Diuresis monitoring: in cases of strong leucocytosis, notable splenomegaly or hyperuricaemia, increased intake of liquids is recommended to obtain abundant diuresis.
It is not advised to use this treatment with live attenuated vaccines, phentoin or fosphenytion (see section 4.5).
