Les enjeux
Le Niger bénéficie actuellement d'une subvention du Mécanisme Transitoire du Fonds mondial et du Nouveau Modèle de financement du Fonds mondial. En dépit des améliorations apportées sur la gestion des approvisionnements et des stocks du VIH/Sida par le pays ces dernières années, il existe encore des défis à relever, dont la collecte et la remontée des données des pharmacies des centres de prise en charge vers le niveau central, ainsi que la coordination et le renforcement du niveau central pour l'analyse et l'exploitation de ces informations. L'analyse des besoins a mis en évidence la pertinence de l'acquisition d'un outil informatique permettant de faciliter la gestion des stocks et le suivi des patients au niveau des sites de prise en charge et des institutions locales, régionales ou nationales. Cet outil pourra servir à la fois pour les intrants du VIH et de la Tuberculose.

Objectifs
Améliorer l'efficience du système d'approvisionnement au Niger : opérationnaliser l'informatisation de la gestion pharmaceutique et logistique.
Activités
Participer à une mission de coordination entre les deux experts au début de la mission
Animer les réunions de concertation avec les acteurs nationaux pour la finalisation du cahier des charges du logiciel
Faire l'inventaire des logiciels existants
Accompagner les acteurs nationaux dans la sélection du logiciel au regard de l'inventaire de l'existant
Accompagner les partenaires dans le suivi des procédures administratives inhérentes à l'acquisition du logiciel
Participer au développement du logiciel sélectionné (aspects techniques pharmacie) et faire le suivi des adaptations requises à chacune des étapes (pré-test, test, post- test)
Echanger avec le concepteur/développeur du logiciel pour son adaptation au cahier des charges
Participer à l'élaboration du matériel de formation des utilisateurs et superviseurs du logiciel

Context
Niger currently benefits from The Global Fund's Transitional Funding Mechanism (TFM) and its new funding model. Despite the improvements made by the country in the last few years in its supply management and HIV/AIDS medication reserves, there are still some challenges to overcome, such as collecting and reporting the data given by the treatment centres' pharmacies back to the central level of management, as well as organising and strengthening the central level in order to analyse and take advantage of this information. A needs assessment shows the call for a computer tool to ease stock management and patient management at treatment centres and local, regional and national institutions. This tool will be used for inputting data for both HIV and tuberculosis.

Objectives
Improve the efficiency of the supply management in Niger; put into action the computerisation of the pharmaceutical and logistical management.
Activities
A teamwork exercise for the two professionals at the start of the project
Host meetings with the national shareholders in order to finalise the software
Complete an inventory of existing software
Help the shareholders select the new software, while regarding the existing software
Guide the partners through the administrative procedures of buying the software
Develop the technical and pharmaceutical aspects of the chosen software and make any required adaptations during any of the stages of development (e.g. pre-testing, testing, post-testing)
Feedback with the software developer regarding the adaptations made
Participate in the creation of training material of the software for users and supervisors
Train the shareholders in the use of the software (both at central level and at pilot sites)
Organise a post-training follow-up including guidance, mentoring and formative supervision (a minimum of 2 follow-up sessions are planned for each pilot site)
Organise test phase workshops and an end of project assessment
Make recommendations on extending the project to other sites
Support the DPHL during the setting-up of workshops by devising and running a software-testing workshop and a training workshop for the 18 users at the 8 pilot prescription centres at central level
Devise and run an assessment workshop during the testing phase
Help the DPHL in the supervision and post-training follow-up of the pilot sites
Expected Results
A piece of software specifically adapted for the situation in Niger and developed using the World Bank funding
The software will be trialled at several treatment centres
The central level and pilot site users will be trained in the use of the software and a post-training follow-up undertaken
A test phase assessment will be carried out and recommendations made in order to expand the use of the software at other sites

Projet Solthis accompagnement professionnelles du sexe, lutte contre VIH sida et IST au Niger
Les enjeux
Ce projet s'inscrit dans le projet global « MAP » (Most at risk populations) financé par la Banque mondiale au Niger, il vise à améliorer l'accès à certaines populations dites « clés » aux services liés à la lutte contre le VIH/Sida et les IST dans le pays.
En Afrique de l'Ouest et du Centre, l'épidémie du VIH est aujourd'hui concentrée dans des populations clés dont les professionnelles du sexe et leurs clients. La réflexion et la mise en place de stratégies spécifiques et adaptées sont essentielles, afin élaborer des programmes d'éducation pour la santé ciblant les besoins spécifiques de ces populations. Un effort tout particulier doit être porté au niveau du dépistage, de l'accès au traitement et de la lutte contre la discrimination dont ces populations font l'objet.

Objectifs
Accroître l'accès des professionnelles du sexe (PS) et de leurs clients aux services liés à la lutte contre le VIH/Sida et les IST dans les régions d'Agadez et de Tillabéry
Activités
Solthis a été retenu pour mettre en œuvre ce projet dans les régions de Tillabéry et d'Agadez, dans le cadre d'un consortium avec l'ONG américaine PSI, et les associations locales ANBEF et Lafia Matassa. Les activités suivantes sont mises en œuvre par le consortium.
Etudes préliminaires
– Analyse documentaire (exploitation des rapports d'études et enquêtes en lien avec les cibles)
– Réalisation d'une cartographie des PS
– Réalisation de focus groupes avec les PS pour finaliser les interventions à mener
– Identification et formation/mise à niveau de relais communautaires (pairs éducateurs) au sein des groupes cibles sur la base de critères de sélection définis avec les bénéficiaires
Prévention et dépistage
– Activités de sensibilisation à destination des PS et de leurs clients : groupes de paroles, causeries éducatives, entretiens individuels, production et dissémination de matériel éducatif (dépliants, affiches)
– Ateliers de sensibilisation vis-à-vis des tenanciers des bars
– Organisation d'un dépistage volontaire des professionnels de sexes et leurs clients;
– Création de points de distribution non traditionnels de préservatifs et ajustement des points de distribution existants (marché de bétail, marchés de nuit, affluences liées au travail saisonniers, sites pétroliers etc…) ;
Renforcement des capacités
– Renforcement des capacités des relais (formations, mise à disposition d'outils)
– Renforcement des capacités des prestataires de soins publics à l'accueil des PS et de leurs clients, à la dispensation du paquet de service et à l'accompagnement vers le traitement et le soin socioéconomique
– Renforcement des capacités des associations locales pour l'accueil des PS
– Coordination du projet avec les divers acteurs ayant un rôle dans les réponses aux besoins exprimés par les PS et leurs clients (autorités administratives, religieuses, coutumières, les représentants de la société civile etc);

Context
This project, which forms part of the World Bank-funded global project MARP (Most At-Risk Populations) in Niger, aims to improve the access for key populations in the country to services used in the fight against HIV/AIDS and sexually transmitted infections (STIs).
Today in Western and Central Africa, sex workers and their clients constitute the key group affected by the HIV epidemic. It is essential to acknowledge the issue and implement specific and adapted strategies in order to develop health-based schemes of education that target the particular needs of these groups. A distinctive approach is needed for the detection and screening of HIV/AIDS, the access to and availability of treatment and the struggle against discrimination that faces these populations.

Objectives
To increase access to HIV/AIDS and STI treatment services for sex workers and their clients in the Agadez and Tillabéri regions of Niger.
Activities
Solthis has been brought on board to implement this project in the Agadez and Tillabéri regions as part of a consortium with the American non-governmental organisation (NGO) PSI (Population Services International), and the local associations ANBEF (Association Nigérienne pour le Bien-Être Familial) and Lafia Matassa. The following activities have been carried out by the consortium:
Preliminary Studies
Analysing documents (using reports, studies and surveys in line with the objectives)
Mapping the sex workers in the areas
Focus groups with sex workers to finalise the actions to be carried out
Identifying and developing community volunteers and peer educators in the centre of the target groups, on the basis of the selection criteria defined by the beneficiaries;
Prevention and detection
Awareness activities with the sex workers and their clients, such as group therapy sessions, educational talks and conferences, individual interviews and the production and distribution of educational material (leaflets, posters)
Awareness workshops concerning bar owners
Organising a voluntary screening for sex workers and their clients
Creating non-traditional distribution points for condoms and modifying existing distribution points (cattle markets, night markets, crowds linked with seasonal workers, oil tankers/rigs, etc.);
Capacity building
Increased capacity to reach communities (training and education, increasing availability of tools and services
Increased capacity of public health providers to accommodate sex workers and their clients, distribute services packages and support with treatment and socioeconomic status and health
Increased capacity of local associations to welcome and help sex workers
Organisation of a project with various actors having a role in responding to the needs shown by the sex workers and their clients (administrative, religious, cultural, traditional, representation of civil society, etc.);

Les enjeux
Le taux de prévalence du VIH au Niger est de 0,4% au sein de la population de 15-49 ans. Il plus élevé en milieu urbain (0,8) qu'en milieu rural (0,2%), et on constate une hétérogénéité selon les régions, Niamey ayant le taux le plus élevé (1,1%). Le taux de couverture des besoins en traitement antirétroviral est estimé à seulement 28%, renvoyant directement au double défi de la poursuite de la décentralisation et de l'augmentation du nombre de dépistage, dans un contexte de ressources humaines, notamment médicale, en sous-nombre. Cette problématique souligne l'importance de la délégation des tâches aux paramédicaux (infirmiers et sages-femmes), telle que prévue au sein du programme officiel initié par l'ULSS, pour assurer la pérennité de la prise en charge des patients séropositifs. Du fait de leur proximité avec la capitale Niamey, les régions de Tillabéri et Dosso représentent un intérêt stratégique en matière de décentralisation afin de décongestionner les hôpitaux de Niamey et rapprocher l'offre de prise en charge des patients contribuant à leur rétention dans le circuit de soins et à leur observance.

Context
The prevalence rate of HIV among 15 to 49 year olds in Niger is 0.4%. It is higher in urban areas (0.8%) than in rural areas (0.2%), although there is some variety depending on the region, with Niamey having the highest rate (1.1%). It is estimated that only 28% receive the necessary antiretroviral treatments, which makes the aim of decentralisation and improved detection of HIV/AIDS a tall task, in terms of human resources, particularly medical, given the shortage of personnel. This problem underlines the importance of delegating tasks to paramedics, nurses and midwives in order to guarantee the continued treatment of HIV-positive people, as was expected in the official programme introduced by ULSS (Unità Locale Socio Sanitaria, an Italian HIV/AIDS organisation). Given their proximity to the capital, Niamey, the regions of Tillabéri and Dosso are of strategic importance to the decentralisation in order to decongest the hospitals in Niamey by being able to treat patients and continue observe them in these regions.
