La lucha contra el dolor que es un compañero innato de la humanidad desde su origen. Primero con un componente mágico, luego con preparaciones artesanales y posteriormente acompañando al método científico de cada época, la búsqueda de la analgesia ideal ha acompañado siempre al hombre.
Se piensa que ya el hombre de cromagnon, utilizaba aplicaciones cutáneas de aceite, grasa de animales e inhalación del humo de diversas sustancias que puede suponerse actuarían como analgésicos.
Las referencias más antiguas a un tratamiento analgésico se encuentren en los papiros de Ebers y Edwing-Smith que con dos siglos de diferencia en sus origenes muestran las tecnicas médicas para tratar el dolor en el Antiguo Egipto. Imhotep, médico y arquitecto conocía el opio, la mandrágora y el cannabis.
Aplicaba para extracciones dentales distintas preparaciones con mezclas de ajo, agua y aceite. Los egipcios comprimían nervios periféricos para insensibilizar zonas determinadas sobre las que actuaban quirúrgicamente.
A pesar de que la mayoría de los historiadores atribuyen a los Incas ser pioneros en el uso de la coca como anestésico local fueron los Aymaras, tribu originaria de una zona que actualmente corresponde a Bolivia los primeros en utilizarla con fines tanto religiosos, como sociales y médicos.
El más famoso médico de la antiguedad china Hua-tuo, estudió y utilizó la acupuntura y la moxibustión como tratamientos médicos analgésicos en la época, todos ellos aplicados en las curas locales de heridas.
En la Antigua Grecia Plinio el viejo pone la Borraga en boca de Homero cuando describía un estado de adormecimiento en La Odisea para la curación de heridas. (Borago Officinalis).
Herodoto refiere como un médico griego calmó el dolor del rey Darío después de fracturarse un tobillo y ser éste reducido por unos médicos egipcios. Dioscórides médico del ejército de Nerón. Utiliza una medicina basada en las plantas. Como hipnóticos y analgésicos utiliza la atropa ,mandrágora, opio y cicuta que usa para distintas intervenciones quirúrgicas.
Avicena ya describe la crioanalgesia y Leonardo da Vinci hace una descripción anatómica de los nervios y la relaciona con el dolor.
Al igual que Descartes aportando la idea de que el dolor viaja por finas hebras o tubos hasta el cerebro.
Ambrosio Paré aplicaba enfriamiento o congelación sobre la zona operatoria como anestésico. James moore llevaba a cabo amputaciones de miembros inferiores comprimiendo el ciático y el nervio femoral.
Se puede considerar la primera anestesia regional moderna la realizada por el oftalmólogo Koller, quien como resultado de su investigación sobre la coca consiguió llevar a cabo una intervención de glaucoma en su clínica vienesa sin que el paciente sufriera dolor durante la intervención.
Unos años antes se había inventado la primera aguja metálica por F. Rynd lo que hizo que comenzara a desarrollarse la anestesia por infiltración. El primer bloqueo del plexo braquial lo realiza Halsted en 1884.
Con el inicio del siglo XX comienza a desarrollarse la anestesia regional con la evolución de la farmacología y el desarrollo y diseño de nuevos anestésicos locales más seguros y carentes de los efectos secundarios que hicieron tan impopular la anestesia regional con cocaína a finales del siglo anterior.
En 1905 aparece la procaína sintetizada por Alfred Einharn que comercializada como Novocaina y que será el anestésico local más importante hasta la década de los años cuarenta.
En 1911 Hirschel describe la primera técnica percutánea sobre el plexo braquial a nivel axilar, posteriormente se comienzan a describir bloqueos como los accesos de Kulenkampff o Winnie.
En 1912 Von Perthes, basándose en los estudios de Galvani del siglo XVIII, describe el uso de un estimulador eléctrico de nervios periféricos adelantando su uso para la localización nerviosa. Éstas experiencias no cristalizan hasta pasado la mitad del siglo en que Yasuda y Magora lo aplican en anestesia regional para la localización de los troncos nerviosos y nervios periféricos a bloquear. En 1920 Gaston Louis Labat publica el libro Lànesthesie Regionale.
En 1971 se diseña el primer ecógrafo que introduce la escala de grises marcando el comienzo de la ecografia como técnica de diagnóstico clínico.
En 1978 La Granje y cols emplearon la ecografía para realizar un bloqueo del plexo braquial supraclavicular identificando mediante doppler la arteria y vena subclavia.
A partir de año 1978 se suceden artículos describiendo distintos bloqueos regionales utilizando la ecografía como técnica de localización de los distintos plexos y distintos abordajes.
En el 94 Kapral y cols publican un artículo usando la ecografía para bloqueo del plexo braquial supraclavicular incluyendo la observación directa de la distribución del fármaco.
Desde entonces la mejora tecnológica de la ecografía junto con la gran aceptación que esta tecnología ha tenido entre los anestesistas que realizan anestesia regional nos ha llevado a la situación en la que nos encontramos actualmente.

From the beginning of human evolution, we have been searching for ways to fight pain. Firstly we turned to magic, then homemade remedies, and subsequently our quest for the perfect painkiller has evolved along with the scientific approaches developed in each time period.
It is believed that early modern humans used skin ointments made from oils and animal fats as analgesics, as well as inhaling the smoke produced by burning different substances.
The earliest references to painkillers can be found in the Ebers and Edwin Smith papyri. These two texts, which were written around two centuries apart, reveal the medical techniques used to treat pain in Ancient Egypt. The doctor and architect Imhotep referred to the analgesic effects of opium, mandrake and cannabis.
For tooth extractions he applied different ointments containing garlic, water and oil. The Egyptians compressed peripheral nerves in order to desensitize the areas where surgery was to be carried out.
Although most historians believe that the first use of coca as a local anesthetic can be attributed to the Incas, the Aymara tribe, which originated in what is now Bolivia, was the first civilization to use the plant for religious, social and medical purposes.

The first modern regional anesthesia can be attributed to the ophthalmologist Koller, who, as a result of his research into the properties of coca , was able to perform glaucoma surgery in his Viennese clinic without causing any pain to the patient.
Some years before, Francis Rynd had invented the first metallic syringe, facilitating the development of infiltration anesthesia. Halsted carried out the first brachial plexus block in 1884.
At the beginning of the 20th century, the evolution of pharmacology was accompanied by the development and production of safer new local anesthetics which did not produce the side effects which made local anesthesia with cocaine so unpopular at the end of the previous century.
In 1905, Alfred Einhorn synthesized procaine, which he patented under the trade name Novocain. This would be the most widely-used local anesthetic until the 1940s.
In 1911, Hirschel described the first percutaneous approach to an axillary brachial plexus block. More techniques were subsequently developed, such as those detailed by Kulenkampff and Winnie.

In 1971, the first scanner with gray-scale technology was introduced, marking the beginning of the use of ultrasound as a diagnostic technique.
In 1978, La Grange and colleagues used ultrasound to carry out a supraclavicular brachial plexus block, identifying the subclavian vein and artery with the Doppler technique.
From 1978 many articles were published detailing different approaches and different regional blocks carried out using ultrasound to locate the different plexus.
In 1994, Kapral and colleagues published an article describing the use of ultrasound to directly visualize the brachial plexus and observe the spread of the local anesthetic in a supraclavicular brachial plexus block.
Since then the technological advances of ultrasound, together with the wide acceptance of this technology among anesthetists who perform regional anesthesia, has brought us to the present situation.
