Au coeur des télécommunications aujourd'hui, le smartphone jouera un rôle central dans la médecine de demain.
Muni d'un appareil photo, d'une caméra, d'un micro, d'une bibliothèque, d'un album photos et de musiques, le smartphone est un concentré de technologies. Doté d'applications, il sert de GPS, de lampe torche, de loupe. Surtout, connecté à un réseau sans fil, il permet de surfer sur Internet, d'envoyer des SMS, des e-mails, et, bien sûr, de téléphoner. Au coeur des télécommunications aujourd'hui, le smartphone jouera un rôle central dans la médecine de demain.
"Du fait des réseaux sans fil, d'Internet, des possibilités de connectivité entre objets, des capacités des bandes passantes, de l'extraordinaire diffusion des smartphones, du cloud computing, cette "informatique dans les nuages" capable de stocker de gigantesques volumes de données, on observe une superconvergence d'outils pouvant être mis au service de la santé", déclare le professeur Eric Topol, cardiologue, spécialiste de génomique et directeur du Scripps Translational Science Institute (La Jolla, Californie). "Nous vivons une révolution numérique qui va bouleverser la médecine", ajoute-t-il avant de faire remarquer qu'on compte "plus de six milliards de téléphones portables dans le monde, plus que de brosses à dents ou de toilettes", dont un milliard de smartphones, un chiffre qui devrait doubler d'ici à 2015.
ÉLECTRODES
Plusieurs dispositifs innovants, réservés au corps médical, sont commercialisés aux Etats-Unis. Conçu par la firme californienne AliveCor, l'un d'eux permet de visualiser l'électrocardiogramme (ECG) du patient sur un smartphone doté d'une application et muni à l'arrière d'un étui contenant deux électrodes plates. Il suffit au patient de tenir horizontalement un iPhone en appuyant d'un doigt sur chaque électrode pour que s'affichent instantanément le tracé de son ECG et la fréquence cardiaque. L'ECG, sans fil, à portée de doigts.
Ce smartphone fonctionne également lorsqu'il est plaqué contre la poitrine du malade. Au bout de dix secondes, le tracé est enregistré et envoyé sur le "cloud", à partir duquel il peut être récupéré à tout moment. L'enregistrement peut durer trente secondes, de une à cinq minutes, ou être continu. L'écran affiche un seul tracé ECG, correspondant à une seule dérivation, contre douze et autant de tracés pour un électrocardiogramme standard.
VENTE DIRECTE AU CONSOMMATEUR?
Ce dispositif a été homologué en décembre 2012 par l'Agence américaine des produits alimentaires et des médicaments (FDA), également chargée des dispositifs médicaux, et a obtenu le marquage "CE" (conformité aux normes européennes) en vue d'une commercialisation en Europe. Vendu 199 dollars (150 euros), il peut servir au cabinet médical à dépister rapidement un trouble du rythme cardiaque. AliveCor entend encourager les médecins à prescrire son dispositif aux patients souffrant d'une pathologie cardiaque qui pourraient, en cas de malaise ou de palpitations, enregistrer leur ECG et l'envoyer par courriel à leur médecin pour interprétation. AliveCor espère que la FDA autorisera la vente directe au consommateur de son dispositif au deuxième trimestre 2013.
Développé par General Electric Healthcare, Vscan, un système d'échographie de poche, remplacera-t-il le bon vieux stéthoscope au chevet du malade ? Pas de doute là-dessus, à en croire Eric Topol, pendant quatorze ans chef de service du département de médecine cardiovasculaire de la Cleveland Clinic (Ohio), qui déclare "ne plus utiliser de stéthoscope depuis trois ans", soit depuis l'avènement d'échocardiographes ultraportables. Spécialement adapté à une imagerie abdominale, obstétricale et cardiaque, le Vscan comporte une sonde reliée par câble à un smartphone sur lequel l'opérateur visualise les images.
DISPOSITIFS PORTATIFS
Cet échographe portatif comporte également un Doppler couleur pour l'imagerie en temps réel du flux sanguin. Le Vscan est commercialisé 7 900 dollars, contre plus de 30 000 dollars pour les échographes standards. Vscan a depuis peu un concurrent : le système développé par MobiSante (Redmond, Etat de Washington) d'un coût de 7 500 dollars, homologué par la FDA début 2011, dans lequel la sonde d'échographie n'est plus reliée par un fil au smartphone. Ces dispositifs portatifs pourraient se révéler utiles en zones rurales ainsi que dans les pays en voie de développement.
En août 2012, la société californienne Sotera Wireless a reçu l'homologation de la FDA pour son système ViSi Mobile, qui envoie toute une série de paramètres vitaux sur un petit écran fixé au poignet du malade. Toutes les données sont transmises en continu au médecin hospitalier sur une tablette ou un smartphone. Le patient, sur son lit d'hôpital, debout ou à domicile, peut être suivi d'aussi près que s'il était en unité de soins intensifs.
PHOTOS ET VIDÉOS
D'autres dispositifs couplés à un smartphone ont été développés à l'usage des professionnels de santé, qui peuvent transmettre les données enregistrées et les stocker dans un cloud. Ainsi, à l'aide d'une optique adaptable à l'appareil photo, le dermatologue agrandira vingt fois l'image d'une lésion cutanée suspecte. De même, équipé d'un adaptateur oculaire, le smartphone de l'ophtalmologiste peut prendre des photos haute définition et des vidéos lors de l'examen à la lampe à fente pour examiner la partie antérieure de l'oeil. Muni d'une application qui analyse le son émis à l'expiration, un smartphone a été transformé en spiromètre par des chercheurs de l'université de Washington pour évaluer la fonction respiratoire.
Cellscope, start-up basée à San Francisco, a transformé le smartphone en otoscope. Un embout connecté à l'appareil photo du téléphone portable rend possible l'examen du tympan avec un important effet loupe. Homologué par la FDA, ce dispositif permet aux parents de prendre des clichés des oreilles de leur enfant et de les envoyer au pédiatre, ce qui peut réduire le nombre de consultations tout en permettant de poser le diagnostic d'otite à distance.
BANDELETTES RÉACTIVES
En juillet 2012 a été présenté LifeWatch, le premier smartphone médical grand public, conçu et développé en Israël. Il suffit de poser ses doigts sur des capteurs pour que s'affichent l'ECG, la fréquence cardiaque et respiratoire et la saturation du sang en oxygène. L'appareil, qui fonctionne sous Android, fait également fonction de glucomètre, le taux de glucose sanguin s'affichant après introduction des bandelettes réactives par le haut du smartphone, qui n'a donc pas besoin d'être branché à un lecteur de glycémie. Lifewatch mesure également la conductivité électrique cutanée qui permet de déterminer la masse graisseuse et le niveau de "stress physiologique", également fonction de la variabilité de la fréquence cardiaque.
Situé à l'arrière du téléphone, un capteur à infrarouges mesure la température corporelle. Ce smartphone dispose également d'applications aidant à surveiller l'alimentation, d'un podomètre et d'une fonction de rappel de prise de médicaments. Envoyé sur le cloud de la société, le tracé ECG est analysé par un algorithme qui peut détecter un éventuel trouble du rythme cardiaque et le notifier directement à l'utilisateur. Ce smartphone s'adresse aux consommateurs désireux de prendre leur santé en main, au sens premier du terme, ainsi qu'aux patients souffrant de pathologies chroniques (hypertension artérielle, diabète). Il a reçu le 6 décembre 2012 le marquage CE et est en attente d'une homologation par la FDA. "Il sera vendu au prix de 600 dollars. Une interface en hébreu, arabe et anglais est d'ores et déjà disponible. La vente sur les marchés américain et européen se fera en partenariat avec des compagnies d'assurances, des organismes payeurs et des réseaux de soins", indique la firme israélienne.
ALGORITHMES
Autre dispositif spécialement développé pour le consommateur soucieux de sa santé et de son bien-être, celui dévoilé en décembre 2012 par la société Scanadu, hébergée par les laboratoires Ames, de la NASA, à Moffett Field, en Californie. Baptisé Scanadu Scout, ce dispositif se présente comme un boîtier de petite taille, connecté par Bluetooth à un smartphone. En attente d'homologation par la FDA, l'appareil sera vendu moins de 150 dollars et devrait utiliser une application fonctionnant sur iOS, Android et Windows.
Posé sur la tempe, ce dispositif permet d'afficher en moins de dix secondes la température corporelle, la fréquence cardiaque et sa variabilité, à partir d'algorithmes interprétant l'activité électrique du coeur. Il affiche aussi le temps de transit du pouls (délai qui sépare l'ouverture de la valve aortique de l'arrivée de l'onde de pouls) et le taux d'oxygène dans le sang (oxymétrie de pouls). Un deuxième dispositif transforme le smartphone en lecteur d'analyse d'urine. Utilisant des cartouches jetables, il permet le diagnostic chez la femme enceinte de complications liées à la grossesse : préclampsie, diabète gestationnel, insuffisance rénale, infection urinaire.
TENSIOMÈTRE SANS FIL
Un troisième dispositif utilise le smartphone pour évaluer un état grippal ou pseudo-grippal. Un test salivaire assure une détection rapide du streptocoque A, des virus grippaux A et B, de l'adénovirus et du virus respiratoire syncytial.
En France, deux sociétés conçoivent et développent des dispositifs médicaux mobiles permettant de mesurer ses propres données physiologiques, de les enregistrer sur un smartphone, de les partager par mail avec son médecin et de les stocker dans un cloud sécurisé. La société Withings commercialise un tensiomètre relié à un smartphone iPhone ou Android. Dans la version américaine de l'Apple Store, cet appareil de suivi de la tension artérielle figurait, en septembre 2012, en troisième position parmi les douze objets connectés les plus vendus aux Etats-Unis, selon le site Proximamobile.fr. En septième position : le tensiomètre sans fil de la société iHealth Lab. Ce système d'automesure de la tension artérielle est conçu pour l'iPod Touch, l'iPhone et l'iPad. Il suffit de poser le baladeur numérique, le smartphone ou la tablette sur la station d'accueil pour commencer une mesure.
AIDE AU DIAGNOSTIC
Utilisé sur le lieu des soins comme microscope, le smartphone pourrait être une aide au diagnostic (HIV, paludisme, tuberculose, anémie). Il pourrait aussi servir de lecteur optique des résultats d'un "laboratoire sur puce", dispositif ultraminiaturisé capable de doser des marqueurs présents dans une goutte de sang, d'urine ou de salive. Muni d'une application, le smartphone pourrait afficher les résultats de tests génomiques réalisables en moins de trente minutes et qui permettent aujourd'hui de dépister, au lit du malade, une susceptibilité génétique à développer un effet indésirable redoutable vis-à-vis d'un médicament (carbamazépine, statines) ou de prédire la non-réponse au traitement envisagé (clopidogrel, interféron pégylé). En France, télécharger son génome sur son smartphone demeure pour l'instant strictement interdit.
Par ailleurs, une tablette PC communiquant en mode bidirectionnel avec un dispositif implanté sous la peau a récemment permis de contrôler la délivrance d'un médicament contre l'ostéoporose et d'améliorer la densité minérale osseuse des patients.
Enfin, l'équipe du professeur Topol vient de développer un système de microscopie capable de détecter les cellules qui se détachent de la paroi interne d'une artère coronaire lors de la rupture de la plaque d'athérome à l'origine d'un infarctus du myocarde.
"En faisant communiquer un smartphone avec un biosenseur de la taille d'un grain de riz capable de détecter ces cellules endothéliales circulantes dont la morphologie du noyau est anormale, il serait possible de recevoir sur son smartphone une alarme de survenue imminente d'un infarctus myocardiaque, avant même de ressentir le moindre symptôme", déclare Eric Topol.
Selon ce spécialiste, "la numérisation de l'être humain permettra de meilleurs soins et l'avènement d'une prévention personnalisée. Elle se fera sous la pression des consommateurs, bien plus qu'à l'initiative de la communauté médicale, par nature conservatrice et peu encline à adopter des outils qui vont bouleverser sa pratique et remettre en question une partie de son pouvoir".

“As a result of wireless networks and the Internet, the possibilities of connectivity between objects, the capacities of bandwidths, the extraordinary diffusion of smartphones, of cloud computing, this “information in the clouds” capable of stocking gigantic volumes of data, one observes a superconvergence of tools capable of being put into use in healthcare” declares Professor Eric Topol, cardiologist, gene specialist and director of the Scripps Translational Science Institute, (La Jolla, California). “We are living in a digital age which is going to turn medicine upside down”, he added before making the remark that we can account for “more than six million cell phones in the world, more than toothbrushes or toilets”, of which one million are smartphones, a number that could double between now and 2015.
ELECTRODES
Several innovative devices, reserved for the medical field, are marketed in the Unites States. Engineered by the California firm, AliveCor, one of these devices can view a patient’s electrocardiogram (ECG) on a smartphone loaded with an application, and equipped at the back with a case containing two flat electrodes. All the patient has to do is to hold an iPhone horizontally, while placing a finger on each electrode so that it instantly shows the ECG reading and the heart rate . The ECG, wirelessly, at your fingertips .
This smartphone functions equally well when it lays flat against the chest of the patient . After about ten seconds, the reading is then recorded and sent on the “cloud”, from which it can be retrieved at any moment. The recording can last thirty seconds, from one to five minutes, or continuously. The screen shows only one ECG reading, corresponding to a single lead, to as many as twelve traces for a standard electrocardiogram.
SOLD DIRECTLY TO THE CONSUMER?
This device, officially approved in December of 2012 by the American Food and Drug Administration (FDA), who is also in charge of medical devices, obtained the designation of “CE” (European Conformity) , in the hopes of developing a widespread European market. Sold for $199 (150 euros), this device can be used in a medical office to quickly assess an arrhythmia . Other devices, coupled to a smartphone, were developed for use by healthcare professionals, which can transmit this recorded data and store it in a ‘cloud’. In this way, with the help of an optical lens adapted to the camera, a dermatologist can enlarge the image of a suspicious cutaneous lesion by twenty times. Likewise, an ophthalmologist’s smartphone, equipped with an ocular adapter, can take high definition photographs and videos during the slit lamp exam in order to examine the anterior part of the eye. When equipped with an application that analyzes the sound emitted with expiration, a smartphone was transformed into a spirometer by some researchers at the University of Washington to evaluate respiratory function.
Cellscope, a San Francisco based start-up company, transformed a smartphone into an otoscope. An end-piece, connected to the camera of the cell phone makes a tympanic, or eardrum exam, possible with an important magnifying effect. This device, approved by the FDA, permits parents to take photos of their children’s ears and send them to the pediatrician, which could reduce the number of office visits by enabling the diagnosis of an ear infection from a distance.
TEST STRIPS
In July of 2012 the first medical smartphone for the general public, LifeWatch, which was conceived and developed in Israel, was introduced . All a person needs to do is to place their fingers on some sensors in order to show the ECG, the heart and respiratory rate, and the blood oxygen saturation level. The device, which functions on an Android, also works as a glucose monitor; the blood sugar level is indicated after the glucose sticks are inserted at the top of the smartphone, and therefore does not need to be attached to a Blood Glucose Meter. LifeWatch also measures the skin’s electrical conductivity that assesses a Body Mass Index (BMI) and of the level of “physiological stress”, both according to the variability of the heart rate.
An infrared sensor, situated at the back of the telephone, measures body temperature. This smartphone also has some applications available that help track a person’s diet , has a pedometer, and an alert to remind a patient to take their medication. Sent on the ‘cloud’, an ECG reading is analyzed by an algorithm that can detect a potential cardiac arrhythmia and directly notify the user with that information. This smartphone markets itself to consumers who want to take their health in their own hands, quite literally , as well as to patients who are suffering from chronic pathologies (high blood pressure, diabetes). On December 6, 2012, this same smartphone received the mark of CE (European Conformity), and is awaiting approval by the FDA. The Israelian firm indicates, “It will be sold at the price of $600. An interface in Hebrew, Arabic, and English is already available. The sale on the American and European markets will be in partnership with insurance companies, claims departments , and care networks”.
ALGORITHMS
Another device was unveiled in December of 2012 by the company Scanadu, hosted by the Ames Laboratories of NASA at Moffett Field in California, which was specially developed for the consumer concerned with health and wellness. This device, called the Scanadu Scout, presents itself as a small case or box that is connected by Bluetooth to a smartphone. In less than ten seconds, while placed on the temple, this device displays the body temperature, heart rate and variability, with the assistance of algorithms that interpret the electric activity of the heart . It also posts the pulse transit rate (the period which separates the opening of the aortic valve from the arrival of the pulse wave signals) and the blood oxygen levels (pulse oximetry). A second device transforms the smartphone into a urinalysis reader. This device allows a pregnant woman to use a disposable cartridge at home to detect complications linked to pregnancy: preeclampsia, gestational diabetes, renal insufficiency, or urinary tract infection.
CORDLESS TENSIOMETER
A third device uses the smartphone to evaluate flu-like or pseudo flu-like symptoms. A saliva test ensures a rapid detection of streptococcus A, the flu viruses A and B, adenovirus and RSV (respiratory syncytial virus).
Two companies in France conceived and developed some mobile medical devices that permit a person to measure personal physiological data, to record it on a smartphone, and to share it via email with their doctor and to store it on a secure cloud. The company, Withings , is marketing a tensiometer connected to an iPhone or Android. In the American version from the Apple Store, this arterial blood pressure monitoring device, appeared in September 2012 in third place among the twelve most commonly sold connected devices in the United States, following the Proximamobile.fr site. In seventh place: the wireless tensiometer from the company iHealth Lab. This automatic arterial blood pressure monitor was developed for the iPod Touch, the iPhone, and the iPad. DIAGNOSTIC AID

Additionally, a PC tablet communicating in a bidirectional mode with a device implanted under the skin, recently allowed for the controlled delivery of an anti-osteoporosis medication and for the improvement of the bone mineral density of patients.
Finally, Professor Topol’s team has just developed a microscopic system capable of detecting cells that have detached from the inner coronary artery wall during the rupture of atheromatous plaque, at the site of a myocardial infarction.
“In making a smartphone able to communicate with a biosensor the size of a grain of rice, which is capable of detecting these circulating endothelial cells that have a morphologically abnormal nucleus, it will be possible to receive an alarm on your smartphone of the imminent onset of a myocardial infarction, even before sensing the smallest symptom. ” declares Eric Topol.
According to this specialist, “the digitization of human beings will permit the best care and the advent of a personalized prevention. It will be facilitated by consumer pressure, much more so than by the initiatives of the medical community, conservative by nature and less inclined to adopt tools that are going to shatter its standard practices and put into question some of its power. ”

Directeur de département à l’Institut Max-Planck d’anthropologie évolutionnaire de Leipzig (Allemagne), le paléoanthropologue Jean-Jacques Hublin est invité par le Collège de France à donner un cycle pluriannuel de conférences, dont la première est prévue mercredi 8 octobre, sur les évolutions récentes de son domaine de recherche et les perspectives qu’elles ouvrent à la communauté scientifique.
En paléoanthropologie, la découverte la plus médiatisée de ces dernières années a été la preuve, apportée en 2010 par les chercheurs de votre institut, d’une hybridation entre néandertaliens et hommes modernes. Comment une telle avancée a-t-elle été possible ?
Il a fallu résoudre d’importants problèmes méthodologiques pour extraire l’ADN fossile et éviter sa contamination par de l’ADN moderne. Mais surtout, la baisse énorme des coûts du séquençage a été cruciale. Lorsque l’on a entrepris, voici presque vingt-cinq ans, de séquencer le génome humain, les estimations du coût étaient telles – environ 3 milliards de dollars – que de nombreux laboratoires ont dû s’associer pour mener à bien le projet. Le séquençage de génome néandertalien a coûté 1 000 fois moins et, quelques années plus tard, les coûts ont encore baissé d’un facteur 1 000 !
Cela dit, cette découverte n’a fait que confirmer ce que beaucoup supposaient. Deux espèces sœurs de mammifères, séparées depuis moins de 2 ou 3 millions d’années, ont toutes les chances de pouvoir s’hybrider, voire de produire une descendance féconde. Il y a de nombreux exemples dans la nature. Les hommes modernes et les néandertaliens s’étant séparés depuis environ un demi-million d’années, la plupart des spécialistes convenaient qu’une hybridation avait pu se produire. La vraie question portait plutôt sur l’ampleur du phénomène : cosmétique ou massif. Les opinions étaient très variées. Pour l’heure, la paléogénétique tranche plutôt en faveur du cosmétique, avec seulement 2 % du génome de populations humaines non africaines actuelles provenant de l’homme de Neandertal.
La génétique a-t-elle encore beaucoup à apporter à la paléoanthropologie ?
Enormément. On commence à peine à étudier le déterminisme génétique de certains caractères. Sur la question de l’hybridation entre hommes modernes et groupes archaïques, le raffinement de l’analyse réserve encore sans doute des surprises. Nous allons comprendre où et quand ces mélanges ont eu lieu. Pour l’heure, nous avons pu seulement analyser l’ADN de deux des premiers habitants modernes de l’Eurasie. L’un, retrouvé en Chine, date de 40 000 ans ; l’autre, en Sibérie occidentale, de 45 000 ans. Ces deux individus avaient déjà le même pourcentage de génome néandertalien que des Eurasiens actuels. Ces données suggèrent un petit nombre d’hybridations au tout début de la dispersion de l’homme moderne hors d’Afrique, puis des interactions réduites avec les populations archaïques remplacées.
L’étude de l’ADN des fossiles peut également nous renseigner sur la démographie des populations anciennes, qui est une question cruciale… En outre, la chute des coûts du séquençage est telle qu’elle permet l’exploration à grande échelle des populations actuelles pour mieux comprendre leur histoire : comment ont-elles interagi les unes avec les autres, comment se relient-elles dans le passé, etc.
La révolution technologique de la paléoanthropologie qui est en cours se réduit-elle à la génétique ?
Non. Tout ce qui relève de l’analyse moléculaire des fossiles est en plein développement. Le dosage des isotopes du carbone, de l’azote ou du soufre contenus dans le collagène fossile nous permet d’obtenir des datations toujours plus précises, mais aussi des informations sur le régime alimentaire des individus. Le dosage de certains éléments, comme le strontium, permet pour sa part de tracer les déplacements des sujets tout au long de leur vie ! Une fenêtre s’ouvre sur la vie quotidienne des populations anciennes, et cette possibilité n’était même pas envisagée voilà seulement quelques années.
Ces nouvelles technologies ne font-elles pas passer les paléontologues au second plan ?
Au contraire. D’abord, il y a eu ces dernières années un énorme effort de recherche sur le terrain et une explosion des découvertes de fossiles. Ensuite, de nouvelles technologies ont permis d’améliorer considérablement leur étude anatomique, en particulier les techniques d’imagerie et la réalité virtuelle.
Il faut bien comprendre que les fossiles humains sont souvent des reliques intouchables. Une fois découverts, ils sont mis au musée et considérés comme des trésors nationaux, ce qui parfois réduit considérablement les possibilités d’accès par les spécialistes. J’ai souvent vu des jeunes chercheurs se voir refuser l’accès à des fossiles sous le prétexte qu’ils étaient en cours de reconstruction ou avaient été prêtés pour une exposition. Parfois même on prétendait avoir perdu les clés du tiroir où ils étaient conservés ! En réalité, il faut bien le dire : ce genre d’obstacle est souvent destiné à empêcher les chercheurs de travailler.
Notre idée a été de créer des avatars numériques de ces fossiles qu’il serait possible d’analyser sans faire courir de risque aux originaux. En 2004, nous avons fait l’acquisition de deux scanners à rayons X de haute précision, que nous avons envoyés en Afrique du Sud, au Kenya, au Maroc, en Croatie, etc., pour produire de tels avatars. Ce n’est pas chose facile : le plus gros d’entre eux pèse 2 tonnes et doit être transporté dans un conteneur en plomb qui lui-même en pèse 10. Nous avons depuis été suivis par de nombreuses équipes. Des milliers de ces copies virtuelles qui peuvent être échangées entre chercheurs ont ainsi été obtenues. L’accès ainsi facilité aux fossiles a permis à un grand nombre de travaux d’être conduits. Ces avatars autorisent des possibilités infinies de démontage, de correction de déformation, de reconstruction de parties manquantes, et surtout d’étude des structures internes.
Les révolutions en cours se limitent-elles aux nouvelles technologies ?
Non. Car nous ne sommes pas seulement des os et de l’ADN ! Il y a encore beaucoup de travail à faire pour intégrer les résultats de disciplines qui ont longtemps été pratiquées de manière indépendante : primatologie, archéologie préhistorique, linguistique et même psychologie. En France, notamment, il y a eu longtemps une séparation entre les recherches sur les aspects culturels et les aspects biologiques de l’évolution humaine. Derrière cette séparation se trouve l’idée que l’homme est le fruit d’une évolution biologique qui lui a conféré un gros cerveau et des capacités cognitives importantes et que, une fois ce cap franchi, toute la suite de son évolution relève uniquement du changement culturel. Cette approche est naïve, car l’évolution humaine est un phénomène profondément bio-culturel. Ainsi, avec la révolution du néolithique et le développement de l’agriculture, les hommes ont commencé à consommer céréales et laitages. Mais cela n’a été possible que grâce à une modification de leur génome. Les variations de la tolérance au lactose et au gluten, dans les populations actuelles, dessinent peu ou prou la carte de foyers de néolithisation ! Au cours des 2 derniers millions d’années, les hommes ont externalisé un certain nombre de fonctions biologiques, c’est-à-dire qu’ils ont économisé de l’énergie en déléguant aux outils et à la technologie des fonctions qui, chez les autres espèces, sont des fonctions biologiques. Pensez par exemple à l’alimentation : la préparation des aliments, la cuisson, permettent d’épargner des fonctions digestives coûteuses. L’énergie qui a ainsi été économisée par l’organisme a pu être mise à profit pour d’autres fonctions, et notamment être allouée au cerveau, qui est organe très exigeant. Au cours de l’histoire récente, ce sont des fonctions cérébrales elles-mêmes – communication symbolique, calcul, mémoire, etc. – qui ont commencé à être externalisées.
Ce lien entre culture et biologie est-il à double sens ?
Oui. Par exemple, les enfants humains naissent à intervalles rapprochés mais mettent beaucoup de temps à devenir indépendants. Fait exceptionnel chez les primates, une mère doit souvent s’occuper de plusieurs enfants en même temps. Ce n’est possible que grâce à de complexes interactions sociales, car ces enfants sont partiellement pris en charge par d’autres adultes et interagissent entre eux, au sein de leur fratrie… Il y a là, sans doute, une des clés de la grande sociabilité de notre espèce.
Nos grandes capacités cognitives sont dépendantes d’une biologie très particulière, notamment en ce qui concerne le développement. Mais c’est aussi parce que nous nous sommes entourés de technologie et de complexité sociale que nous pouvons, en quelque sorte, nous offrir le type de biologie qui est le nôtre.

The paleoanthropologist Jean-Jacques Hublin, Department Chair at the Max-Planck Institute for Evolutionary Anthropology of Leipzig (Germany), was invited by the Collège de France to give a multi-year lecture, the first of which is scheduled on Wednesday, October 8th, on the recent developments from his line of research and the prospects that they are bringing to the scientific community.
Foucart: In paleoanthropology, the most highly publicized discovery of the past few years was the proof, given in 2010 by the researchers at your institute, of a hybridization between Neanderthals and modern man. How was such an advancement possible?
Hublin: It was necessary to resolve some important methodological problems to extract the fossil DNA and avoid its contamination by the modern DNA. More importantly, the enormous decrease in the costs of sequencing was crucial. When we undertook it, right here almost twenty-five year ago, the sequencing the human genome, the estimates of the cost were so much – around 3 billion dollars – that numerous laboratories had to partner with each other in order to conduct the project well. The sequencing of the Neanderthal genome cost 1,000 times less and, some years later, those costs decreased again by a factor of 1,000!
That being said, this discovery did nothing but confirm what many believed. Two mammalian sister species, separated by at least 2 or 3 million years, had every chance of having hybridized with each other, ultimately producing prolific descendants. There are numerous examples in nature. Modern man and the Neanderthals being separated from each other by around a half-million years, the majority of specialists agree that a hybridization could have occurred. The real question focused rather on the extent of the phenomenon: cosmetic or structural . Opinions were quite varied. For the time being, the paleogenetic bracket is rather in favor of the cosmetic, with only 2% of the genome of actual non-African human populations originating from the Neanderthal man.
Foucart: Does genetics still have a lot to bring to the field of paleoanthropology?
Hublin: A great deal. We are just beginning to study the genetic determination of certain characteristics. Considering the question of the hybridization between modern man and archaic groups, the refinement of the analysis without a doubt brings some surprises. We are going to understand where and when these genetic combinations took place. At the time being, we have only been able to analyze the DNA of two of the first early Eurasian ancestors. The first one, found in China, is around 40,000 years old, the other one, in Western Siberia, is about 45,000 years old. These two individuals already had the same percentage of Neanderthal genome as modern day Eurasians. Such data suggests a small number of hybridizations at the beginning of the dispersion of modern man out of Africa, then some reduced interactions with the replaced ancient populations.
The study of the DNA of some fossils can equally tell us something of the demographics of the ancient populations, which is a crucial question...In other words, the drop in the costs of sequencing is such that it allows the exploration on a large scale, of the modern populations to better understand their history: how they interacted with each other, how they were connected with each other in the past, etc.
Foucart: Is the current technological revolution of paleoanthropology reduced to genetics?
Hublin: No. Everything coming from the molecular analysis of the fossils is developing quickly. Isotope analyses of carbon, nitrogen and sulfur contained in the fossil collagen allow us to obtain even more precise dating, but also some information about the diets of these individuals. The correct proportioning of elements such at strontium, for its part allows us to trace the movements of the subjects throughout their lives! A window is opening itself onto the daily life of the ancient populations, and this possibility couldn’t even be fathomed before a few years ago.
Foucart: Do these new technologies not put paleontologists in the background?
Hublin: On the contrary. First of all, for the past few years there has been an enormous research effort in this area and an explosion of fossil discoveries. Then, some new technologies have allowed for a considerable improvement in their anatomical study, particularly in the techniques of imaging and virtual reality. It is really necessary to understand that human fossils are often untouchable relics. Once discovered, they are placed in a museum and considered as national treasures, which often considerably reduces the possibilities of access by specialists. I have often seen young researchers be refused access to some fossils under the pretext that they were undergoing reconstruction or have been borrowed for an exhibit. Sometimes people have even pretended to have lost the keys to the drawer where they were conserved! In reality, it really must be said: this sort of obstacle is often destined to prevent the researchers from working.
Our idea was to create some numeric avatars of these fossils, which could be analyzed without running any risk to the originals. In 2004, we acquired two high-precision X-ray scanners, which we have shipped to South Africa, Kenya, Morocco, Croatia, etc., in order to produce such avatars. This is not an easy thing to do: the heaviest between them weighs 2 tons and must be transported in a lead container which itself weighs 10. Numerous teams have since followed us. Thousands of these virtual copies, which can be exchanged among researchers, were obtained in this manner. In this way, the easy access to the fossils allowed a large number of workers to be conduits. These avatars allow access to infinite possibilities of deconstruction, of correction of deformation, of reconstruction of missing parts, and of course of the study of internal structures.
Foucart: Do these revolutions taking place limit themselves to new technologies?
Hublin: No. Because we are not only bones and DNA! There is still a lot of work to be done to integrate the results from disciplines that have long been practiced in an independent fashion: primatology, prehistoric archeology, linguistics and even psychology. In France, notably, there has long been a separation between research on the cultural aspects and the biological aspects of human evolution. Behind this separation is the idea that man is the fruit of a biological evolution which imparted him with a large brain and some important cognitive capacities and that, once they reach a certain threshold, all of a sudden his evolution is attributable to cultural changes. This approach is naïve, because human evolution is a profoundly bio-cultural phenomenon. In this way, with the Neolithic revolution and the development of agriculture, man began to consume grains and dairy. But this was only possible thanks to a modification of their genome. The variations of the tolerance to lactose and gluten in today’s populations more or less draw a map to the Neolithic home! Over the course of the last 2 million years, man externalized a certain number of biological functions, that is to say that they economized energy by delegating to tools and technology some functions which, with other species, are biological functions. Take diet for example: the preparation of food, cooking, which spared costly digestive functions. In this way, the energy that was saved by the organism could be put to use for other functions, and notably to be allocated to the brain, which is a very demanding organ. In the course of recent history, these are brain functions themselves – symbolic communication, calculation, memory, etc. – which began to be externalized.
Foucart: This link between culture and biology, is there a double meaning?
Hublin: Yes. For example, human children are born in short intervals but take a lot of time to become independent. This fact is exceptional in the primate world; a mother often must herself take care of several children at the same time. This is only possible thanks to complex social interactions, because these children are put partially in the care of other adults and interact among them and their siblings...without a doubt therein lies one of the keys to the large sociability of our species.
Our large cognitive capacities are dependent on a very particular biology, most notably concerning development. But it’s also because we surround ourselves with technology and social complexity that we can, to some extent, give ourselves the type of biology, that is our own.
