Dissolve about 40.00 mg AD 1 BR reference standard, accurately weighed, in 0.001N hydrochloric acid to produce 100.0 ml.
Test solution:
Mix together the contents of 5 ampoules. Dilute 2.00 ml of the resulting solution to 100.0 ml with 0.001N hydrochloric acid.
System Suitability Solution:
Dissolve about 10 mg of BA 790 BR reference standard in 0.001N hydrochloric acid to produce 25.0 ml. Dilute 1.00 ml of the resulting solution, together with 40 mg of AD 1 BR reference standard in 0.001N hydrochloric acid to produce 100.0 ml.

Column: LiChrosorb RP-select B, 5 µm, 125 x 4 mm or LiChrospher RP-select B, 5 µm, 125 x 4 mm

November 2011 – Media reports in Germany are currently questioning the efficacy and safety of the novel anticoagulant dabigatran etexilate (Pradaxa®) for patients with atrial fibrillation (AF). Boehringer Ingelheim expressly rejects these reports. “The efficacy (for 150mg bid) and the safety profile of Pradaxa® (for 150mg and 110mg bid) with regards to life-threatening and intracranial bleeds are clearly superior to the current standard therapy with warfarin and have been impressively demonstrated in a comprehensive clinical trial in over 18.000 patients with AF,” said Professor Klaus Dugi, Corporate Senior Vice President Medicine at Boehringer Ingelheim.

Following a single intravenous dose of [14C]alendronate, approximately 50 % of the radioactivity was excreted in the urine within 72 hours and little or no radioactivity was recovered in the faeces. Following a single 10 mg intravenous dose, the renal clearance of alendronate was 71 ml/min, and systemic clearance did not exceed 200 ml/min. Plasma concentrations fell by more than 95 % within six hours following intravenous administration. The terminal half-life in humans is estimated to exceed ten years, reflecting release of alendronate from the skeleton. Alendronate is not excreted through the acidic or basic transport systems of the kidney in rats, and thus it is not anticipated to interfere with the excretion of other medicinal products by those systems in humans.

1. DENOMINATION DU MEDICAMENT
ISOSUPRA LIDOSE 8 mg, gélules.
2. COMPOSITION QUALITATIVE ET QUANTITATIVE
Chaque gélule de Isosupra Lidose contient 8 mg d’isotretinoin (INN). Pour la liste complète des excipients, voir rubrique 6.1.
3. FORME PHARMACEUTIQUE
Gélule.
Gélule orange renfermant une pâte cireuse orangée.
4. DONNEES CLINIQUES
4.1 Indications thérapeutiques
Acnés sévères (telles qu’acné nodulaire, acné conglobata ou acné susceptible d’entraîner des cicatrices définitives) résistantes à des cures appropriées de traitement classique comportant des antibiotiques systémiques et un traitement topique.
4.2 Posologie et mode d’administration
L’isotretinoin doit être uniquement prescrite par ou sous la surveillance de médecins ayant l’expérience de l’utilisation des rétinoïdes systémiques dans le traitement de l’acné sévère ainsi qu’une parfaite connaissance des risques de l’isotretinoin et de la surveillance qu’elle impose.
Les gélules doivent être avalées au cours des repas, en une ou deux prises par jour.
Adolescents, adultes et personnes âgées :
Le traitement par isotretinoin doit être débuté à la posologie de 0,4 mg/kg/jour.
La réponse thérapeutique à l’isotretinoin et certains des effets indésirables sont dose dépendant et varient
d’un patient à l’autre. Cela nécessite un ajustement individuel de la dose au cours du traitement. Pour la
plupart des patients, la dose se situe entre 0,4 et 0,8 mg/kg/jour.
Les taux de rémission prolongée et de rechute après une cure d’isotretinoin dépendent plus de la dose cumulée totale que de la durée de traitement ou de la posologie quotidienne. Il a été démontré que la poursuite du traitement au-delà d’une dose cumulée de l’ordre de 120 à 150 mg/kg n’entraînant aucun bénéfice supplémentaire notable. La durée de traitement dépend de la dose quotidienne individuelle. Une cure de traitement d’une durée de 16 à 24 semaines suffit habituellement à atteindre la rémission.
Chez la majorité des patients, une guérison complète de l’acné est obtenue après une seule cure. En cas de rechute confirmée, une nouvelle cure d’isotretinoin peut être envisagée avec la même posologie quotidienne et la même dose thérapeutique cumulée. Comme l’amélioration de l’acné peut se poursuivre jusqu’à 8 semaines après l’arrêt du traitement, une nouvelle cure avant la fin de ce délai ne doit pas être envisagée.
Patients en insuffisance rénale sévère :
Chez les patients en insuffisance rénale sévère, le traitement doit être initié à une dose plus faible (ex : 8 mg/jour). La posologie sera ensuite augmentée progressivement, jusqu’à 0,8 mg mg/kg/jour, ou jusqu’à la dose maximale tolérée par le patient. (voir section 4.4. « Mises en garde spéciales et précautions particulières d’emploi »).
Enfants :
L’isotretinoin n’est pas indiquée dans le traitement de l’acné pré pubertaire et n’est pas recommandée
chez l’enfant de moins de 12 ans.

4. КЛИНИЧЕСКИЕ ХАРАКТЕРИСТИКИ
4.1 Терапевтические показания
Тяжелые формы угревой болезни (узелково-кистозные, конглобатные угри или угри с риском образования рубцов), резистентные к методам традиционной терапии, включающим назначение системных антибиотиков и местное лечение.
4.2 Способ применения и дозы
Изотретиноин должен назначать врач, имеющий опыт применения системных ретиноидов при тяжелых формах угревой болезни и осведомленный о рисках, связанных с применением изотретиноина, а также о необходимости медицинского наблюдения при применении препарата.
Капсулы принимают внутрь во время еды один или два раза в день.
Подростки, взрослые и лица пожилого возраста:
Терапию изотретиноином начинают с дозы 0,4 мг/кг/сут.
Терапевтическая эффективность изотретиноина и некоторые нежелательные явления носят дозозависимый характер и варьируют у разных пациентов. Это диктует необходимость индивидуального подбора дозы в ходе лечения. Для большинства пациентов доза составляет от 0,4 до 0,8 мг/кг/сут. Частота длительной ремиссии и рецидивов после курса изотретиноина зависит в большей степени от кумулятивной курсовой дозы, чем от продолжительности терапии и суточной дозы. Доказано, что продолжение терапии после превышения курсовой дозы порядка 120-150 мг/кг не приводит к какому-либо заметному дальнейшему улучшению. Продолжительность терапии зависит от индивидуальной суточной дозы. Ремиссия достигается обычно за 16-24 недель.
У большинства пациентов полной ремиссии угревой болезни удается достичь после однократного курса терапии. При подтвержденном рецидиве показан повторный курс изотретиноина в той же суточной и курсовой дозе, что и первый. Поскольку улучшение может продолжаться вплоть до 8 недель после отмены препарата, повторный курс не назначают до истечения этого срока.
Почечная недостаточность тяжелой степени:
При почечной недостаточности тяжелой степени лечение начинают с меньшей дозы (напр., 8 мг/сут.). Далее дозу постепенно увеличивают до 0,8 мг/кг/сут. или до максимально переносимой (см. раздел 4.4 «Особые указания и меры предосторожности»).
Дети:
Изотретиноин не показан для лечения угревой болезни в препубертатном периоде и не рекомендуется к применению у детей до 12 лет.

Bio
Most of my family are healthcare professionals who did not leave their work at work but often discussed professional issues at home. I grew up surrounded by medical books and medical talks but instead of following our family trade I chose the path of a wordsmith and graduated from Moscow State Linguistic University.
My first translation experience was helping my relatives’ colleagues. They were skilled doctors but to keep their knowledge up to date they had to read medical journals in English. I helped them with my knowledge of foreign languages and they shared with me their knowledge of medicine.
Later on, when I began working with translation agencies, I mostly dealt with documents from pharmaceutical companies concerning marketing authorization of drug products, pharmacovigilance and clinical trials. This soon became my specialist subject area. I have been translating texts aimed both at patients and doctors so I know how to adjust my style according to the target audience.
I am also interested in the beauty industry and have considerable experience in translating those marketing texts, from cosmeceuticals used to treat skin problems such as psoriasis, atopic dermatitis or acne to high end designer fragrances.
