3 Zusammenfassung und Diskussion
Die Behandlungsmethode der „angeleiteten Eltern-Säuglings-Übungssitzungen“, Eltern gegenüber „Baby-Lese-Stunden“ genannt, wurde in der Beratungsstelle „MenschensKind“ im Rahmen der „Sprechstunde für Eltern mit Babys, die viel schreien“ entwickelt und bisher bei ca. 140 Säuglingen und ihren Eltern angewendet. Das Ziel der Sitzungen besteht darin, Eltern zu helfen, die Signale ihres Babys entwicklungspsychologisch angemessen zu interpretieren und zu beantworten. Dies wird in den Stunden geübt: ist das Kind in einem aufmerksamen und ruhigen Verhaltenszustand, wird vorgeschlagen, diese „kostbaren“, weil bei sehr jungen Säuglingen kurzen Phasen zu nutzen und mit ihm wechselseitig belohnende Zwiegespräche zu führen (Papoušek u. Papoušek 1987). Zeigt es die ersten Anzeichen einer Veränderung seines Verhaltenszustands in Richtung vermehrter motorischer Unruhe und Quengeln, werden die Eltern angeleitet, Hypothesen darüber zu bilden, was das Baby damit zum Ausdruck bringen möchte, und ganz spezifisch auf seine vermuteten Bedürfnisse zu reagieren. Alle Hypothesen der Eltern werden ernstgenommen und überprüft. An der Reaktion des Babys wird deutlich werden, ob die Hypothese stimmt oder nicht.
Der Termin für „Baby-Lese-Stunden“ wird nach Möglichkeit zu einer Zeit gelegt, wo die Wahrscheinlichkeit, dass das Kind – zumindest einen Teil der Zeit – aus „vollem Halse“ schreit, hoch ist. D.h. in dieser Behandlungsform ist das exzessive Schreien kein unerwünschtes Nebenprodukt, das – da es das Reden erschwert – die Beratung stört, sondern ein Verhaltenszustand, der explizit in Erscheinung treten soll, denn nur wenn das Baby das für Eltern schwierige Verhalten zeigt, kann im Hier-und-Jetzt ein anderer Umgang damit erprobt werden.
Die Erfahrungen mit der vorliegenden Behandlungsmethode liefern eine klinische Bestätigung des postulierten Zusammenhangs zwischen exzessivem Schreien und Problemen der Schlaf-Wach-Organisation dieser Kinder (Becker et al. 1998; Hurry et al. 1991; Papoušek u. Papoušek 1996; Warren 1998): Ein Grossteil der Babys schreit, weil er übermüdet ist und nicht einschlafen kann oder nach kurzer Zeit wieder aufwacht, weil der Übergang in den Tiefschlaf noch nicht gelingt. Deshalb liegt der Interventionsfokus in den „Baby-Lese-Stunden“ zunächst meist auf einer Stabilisierung der Schlaforganisation. Dies entspricht dem Ansatz von Warren et al. (1988). Verbessert sich das Schlafverhalten, berichten die Eltern in den Folgesitzungen häufig auch von wechselseitig belohnenderen Interaktionen mit Ihrem Kind im Wachzustand. Papoušek und Papoušek (1996) heben hervor, dass die Schlaforganisation auch über eine Verbesserung der Spielinteraktionen beeinflusst werden kann.
In den „Baby-Lese-Stunden“ sehen sich die Eltern fast immer mit der Entscheidung konfrontiert, wie ihr Baby einschlafen soll: d.h. ob dies allein durch ihre eigene Aktivität geschehen soll (durch Stillen, Herumtragen usw.), oder ob sie bereits so früh damit beginnen wollen, die selbstregulativen Fähigkeiten ihres Kindes zu unterstützen, so dass es – zunächst in ihrer Anwesenheit – nach und nach lernen kann, allein in den Schlaf zu finden. Dies ist für viele Eltern keine einfache Entscheidung und wird auch von Fachleuten kontrovers diskutiert (z.B. McKenna et al. 1990). An dieser Stelle soll explizit betont werden, dass in den „Baby-Lese-Stunden“ von therapeutischer Seite diesbezüglich keine Empfehlung gegeben wird. Wichtig ist, dass Eltern einen Weg finden, mit dem sie sich gefühlsmässig gut arrangieren können, so dass das Kind, welches sehr sensibel auf den affektiven Gehalt von Mitteilungen reagiert, möglichst wenig ambivalente Botschaften erhält. Es kann vermutet werden, dass der wesentliche „Wirkfactor“ der „angeleiteten Eltern-Säuglings-Übungssitzungen“ darin liegt, dass die Eltern im Kontext einer positiven therapeutischen Beziehung die konkrete Erfahrung machen, dass sie die Mitteilungen ihres Kind verstehen und ihr zuvor als unstillbar schreiend wahrgenommenes Baby selbst beruhigen können.
Die klinischen Erfahrungen zeigen, dass ein bis zwei „Baby-Lese-Stunden“ zu einer deutlichen Entspannung der Symptomatik führen können. Eine vollständige Besserung ist bei vielen Babys aufgrund der reifungsbedingten Anteile des exzessiven Schreiens in den ersten Lebensmonaten kaum zu erwarten. Bei einem kleineren Teil der Familien wird schnell deutlich, dass die Methode nicht ausreicht, z.B. weil innere elterliche Konflikte einer einigermassen realitätsangemessenen Interpretation der kindlichen Signale im Wege stehen, sofort eine negative Übertragung entsteht und/oder schwierige sozio-ökonomische Probleme grosse Anforderungen an die Eltern stellen. In diesen Fällen erweist es sich als notwendig, alternativ oder ergänzend andere Behandlungsformen hinzuzuziehen. Bei den sogenannten „Gespenstern im Kinderzimmer“ können dies psychotherapeutisch orientierte Verfahren wie die Eltern-Säuglings-Psychotherapie sein (Fraiberg 1980; Brazelton u. Cramer 1991; Barth 1998). Liegt eine Häufung psychosozialer und medizinischer Risikobedingungen vor, muss auch die Einbeziehung anderer Institutionen und Berufsgruppen erwogen werden (vgl. Stellungnahme der Gesellschaft für seelische Gesundheit in der frühen Kindheit).
„Baby-Lese-Stunden“ sind ein niedrigschwelliges Behandlungsangebot. Wenn Eltern bei der telefonischen Anmeldung erfahren, dass es dabei zunächst einmal nur darum geht, herauszufinden, was das Kind mit seinem Quengeln und Schreien eigentlich zum Ausdruck bringen möchte, um daraus dann elterliche Interventionen abzuleiten, sind sie in der Regel sehr angetan. Ein solches, sich an den kindlichen Signalen orientierendes Vorgehen steht in deutlichem Gegensatz zu den verbreiteten unspezifischen Beruhigungsbemühungen, wie ein Baby, das quengelt oder schreit, immer zu stillen/füttern, herumzutragen oder im Kinderwagen bzw. Auto spazierenzufahren. Ein Hinweis für die hohe Akzeptanz dieses Ansatzes kann darin gesehen werden, dass Neuanmeldungen bei „MenschensKind“ inzwischen zu ca. einem Viertel aus Eltern mit exzessiv schreienden Säuglingen bestehen.

3 Summary and Discussion
The treatment approach of “guided parent-infant-training sessions” (called “baby reading hours” for parents) was developed in the “MenschensKind” clinic within the framework of the “Consultation hour for parents with babies that cry a lot” and has been applied thus far with some 140 infants and their parents. The goal of the sessions consists of helping parents to interpret and answer the signals of their baby in a manner appropriate to its developmental psychological state. This is then practiced in the sessions: if the child is in an attentive and calm behavioral state, it is suggested to use this “precious” time, since it is so short for small babies, to hold reciprocally rewarding dialogues with it (Papoušek and Papoušek 1987). If the first signs of a change in its behavioral state show in the direction of increased physical agitation and whining, the parents are instructed to form hypotheses about what the baby would like to express through this and to react very specifically to its supposed needs. All hypotheses of the parents are taken seriously and examined. The reaction of the baby will make it clear whether the hypotheses are correct or not.
The appointment for the “Baby reading hours” is then, if possible, arranged at a time when the probability is high that the child will, at least for part of the time, cry “at the top of its lungs”. This means that in this form of treatment, the excessive crying is not an undesired side effect that – since it makes talking more difficult – upsets the consultation, rather a behavioral state that should explicitly appear, since only when the baby shows the difficult behavior for the parents, can a different handling of it be tested out in the here-and-now.
The experiences with the present treatment method provide a clinical confirmation of the postulated correlation between excessive crying and problems with the sleeping-waking organization of these children (Becker et al. 1998; Hurry et al. 1991; Papoušek and Papoušek 1996; Warren 1998): a majority of the babies cry because they are overtired and cannot fall asleep or wake back up after a short time, since the crossover into the deep sleep state is still not succeeding. Therefore, the focus of the intervention in the “Baby reading hours” lies initially for the most part on a stabilization of the sleeping-waking organization. This corresponds with the approach of Warren et al. (1988). If the sleep behavior improves, the parents frequently also report in the following sessions of reciprocally more rewarding interactions with their child in the waking state. Papoušek and Papoušek (1996) emphasize that the sleeping-waking organization can also be influenced through an improvement in play interactions.
In the “Baby reading hours”, parents almost always see themselves confronted with the decision as to how their baby should fall asleep: this means whether this should happen alone through their own activities (through feeding, carrying around, etc.), or whether they already want to begin this early to support the self-regulatory capabilities of their child so that it – initially in their presence – gradually can learn how to fall asleep alone. This is not an easy decision for many parents and is also the subject of controversial discussion amongst experts in the field (e.g. McKenna et al. 1990). At this point, it should be explicitly emphasized in the “Baby reading hours” on the side of the therapist that no recommendations are made in regards to this. It is important that parents find a method to which they can emotionally well adjust so that the child, which reacts very sensitively to the affective contact of signals, receives as few ambivalent messages as possible. It can be surmised that the essential “operative factor” of the “guided parent-infant-training sessions” lies therein, that the parents have a concrete experience in the context of a positive therapeutic relationship that they understand the signals of their child and they can calm their until now perceived as unpacifiably crying baby themselves.
The clinical experiences demonstrate that one to two “Baby reading hours” can lead to a clear relief of the symptoms. A complete recovery is hardly to be expected with many babies due to the parts of the excessive crying related to the maturing process during the first months of life. With a smaller fraction of the families, it quickly becomes clear that this method is not sufficient, because, for instance, internal parental conflicts stand in the way of a reasonably realistic interpretation of the child’s signals, a negative transference forms immediately and/or difficult socio-economic problems place significant requirements on the parents. In these cases it proves necessary to call in alternative or complementary other forms of treatment. With so-called “ghosts in the playroom”, these can be psychotherapeutic-oriented processes like parent-infant psychotherapy (Fraiberg 1980; Brazelton and Cramer 1991; Barth 1998). If an accumulation of psychosocial and medical risk factors exists, the inclusion of other institutions and professions must also be considered (compare the statement of the Society for Mental Health in Early Childhood).
“Baby reading hours” are a low-threshold treatment offering. If parents learn during the phone registration that it initially only concerns finding out what the child would actually like to express with its whining and crying in order to infer parental interventions out of this, they are generally very taken with the concept. Such an approach, oriented around the child’s signals, stands in clear opposition to the common, untargeted pacification efforts, such as always feeding a crying or whining baby, carrying it around, or driving it around in a stroller or a car. An indication of the high acceptance of this approach can be seen in that up to around one-fourth of new registrations at “MenschensKind” in the meantime are made up of parents with excessively crying babies.
