Note: in NY, unlike every other US state, the "Supreme Court" is the court of first instance, not the court of final appeal.

These are simplified forms that make it easy to file lawsuits and provide uniformity and simplicity in court administration.

Annabelle, your translation is correct, but this is not "intervention" in the sense you describe in your explanation. You are talking about additional plaintiffs or defendants joining ("intervening in") the case. That's not what these forms are about; they're just about how to file a case in the first place. It's a request that the court intervene in resolving a dispute, because the parties haven't resolved it themselves.

En droit français, il existe:
- l'intervention volontaire: un tiers demande à se joindre à une instance en cours.
- l'intervention forcée: un plaignant ou un défenseur demande qu'un tiers soit joint à l'instance afin qu'il puisse répondre du jugement
- l'appel en garantie: un tiers est appelé à l'instance en garantie d'un paiement.
