Examen digestif : Abdomen souple, depressible et indolore. Orifices herniaires libres. Pas de defense ou de contracture. Pas de masse palpée. Absence de signes d'hepatopathie chroniques. Absence d'ictere cutané.
Examen pulmonaire: murmure vesiculaire bilateral et symétrique sans bruits sur-ajoutées. Eupnée en air ambiant.
Examen cardio-vasculaire : Bruits du coeur réguliers sans souffle. Mollets souples et indolores. Absence de signes d'insuffisance cardiaque droite ou gauche. Absence de signes d'insuffisance circulatoire peripherique.

Neurological examination: Conscious and oriented in time and space. No sensory-motor deficits in four limbs.
Skin examination: Erythematous papules on the lower limbs and necrotic ulceration on the left thigh (lesions appearing after the injection administrated as part of X).

Cette lésion s'étend jusqu'a l'artère interventriculaire antérieure moyenne. Le diamètre de référence est de 3 mm a ce niveau. Le flux coronaire est normal (TIMI 3). L'artère interventriculaire antérieure moyenne présente une sténose significative (50-70%).
ANGIOPLASTIE CORONAIRE
Mise en place d'un ballon NC EMERGE 3,5 X 12 mm au niveau de l'artère coronaire droite proximale pour prédilater la lésion. Une inflation est réalisée a une pression maximale de 12 ATM pour une durée totale de 30 sec.

1. The IASD System II is placed by an interventional cardiologist using a standard non-surgical procedure.
2. During the procedure, a needle is used to create a very small communication in the heart wall between the two upper chambers.
3. The implant is then placed in the opening to allow blood to flow between the two chambers and reduce the pressure in the left side of the heart, which contributes to the symptoms of heart failure.

Carglumic acid is an amino acid and a structural analogue of NAG. NAGS, a mitochondrial enzyme, catalyzes the formation of NAG, an essential, allosteric activator of CPS1, the first enzyme of the urea cycle. Carglumic acid acts as a replacement for NAG in NAGS deficiency patients by activating CPS1.
In patients with NAG deficiency, carglumic acid was shown to induce a rapid normalisation of plasma ammonia levels, usually within 24 hours. When the treatment was instituted before any permanent brain damage, patients exhibited normal growth and psychomotor development.
Other studies have been conducted in rats under different experimental conditions leading to increased ammonia availability (starvation, protein-free or high-protein diet). Carglumic acid was shown to decrease blood ammonia levels and increase urea levels in blood and urine, whereas the liver content of carbamoyl phosphate synthetase activators was significantly increased.
