If you really want to conjugate all the verbs," sortit et dit" ; personally I'd be tempted to say " quand la vieille sage-femme sortit pour dire à mon père qu'il avait une belle petite fille"
In fact I'd have thought it was 'had' that would cause more uncertainty. But if you put it into direct speech : "Vous avez une belle petite fille", then it is clear that the imperfect is necessary.

Un disagree me semble un peu fort: une de mes propositions est identique à la vôtre! :-) J'ai toujours appris qu'un "et" était maladroit pour traduire "went/went in /came/came out and V".Lien de cause implicite, a mon avis - pourquoi donc est-elle sortie?
