"Tapering off" está corretíssimo e é muito usado. Mas não é mais formal que "gradual discontinuation". Além disso, "tapering off" pode ser usado para milhões de situações, como "my cold is tapering off", "this song doesn´t have a clean ending, it tapers off", etc. Também me parece que pacientes e leigos usam mais a expressão "tapering" e médicos e artigos científicos "discontinuation". Um exemplo é esse artigo de Harvard dirigido a pacientes. Quando se dirige ao leitor, ao leigo, o autor usa "tapering", mas sempre explicando que "tapering" é a "gradual discontinuation".

Pessoal, liguei para o médico e ele me disse que a "programação de retirada", em português, já indica que é uma diminuição da dosagem até que nenhuma dose seja mais dada (como explicou o Oliver ao sugerir "weaned off"). Entretanto, só encontrei as expressões "be weaned off", "come off", "be taken off" em textos de leigos ou para leigos. O PubMed apresenta inúmeras ocorrências de "gradual discontinuation". É o que vou utilizar. Obrigada a todos

só tirando uma dúvida. Se a retirada é gradual, não deveria haver uma forma/termo que esclarecesse também 'gradual'?

Sugestão.

Me parece que "withdrawal" é algo mais brusco, não-programado. "Discontinuation" serve para todos os casos, tanto a parada programada, a não-programada e a síndrome que acomete o paciente.
Já "withdrawal" parece se aplicar mais à síndrome, mais à retirada brusca (que não parece ser o caso em tela).

This is the idiomatic way of saying it in US English. There may be a more formal way which perhaps our colleagues who work in the medical field would care to contribute here.

(You might think that "medication withdrawal" per se would be idiomatic in English, but it is not.)
