I assume this is for an American readership - otherwise I'd say excess instead of co-pay.

"Prticipação obrigatória do segurado" is not a good choice of term in my opinion, as they're both mandatory participations.
I assume this is for an American readership - otherwise I'd say excess instead of co-pay.
