I am translating this for a friend and I am not very experienced in medical translation. It's the hospital report he received after a visit to the emergency room because of a persistent cough and difficuty breathing:

Ao exame objetivo, doente consciente, colaborante sem SDR e sem tiragens.

AC-S1 e S2+ rítmico, não ouço sopros

I am a Spanish-English medical interpreter. Usually one of the first things they assess in an emergency room is if the patient has shortness of breath. Although it is not quite the literal translation of Teresa's "Sinais de Dificuldade Respiratoria" it would be the term most commonly used in an emergency room setting.
