I'm very glad you think so, since that's what I suggested earlier: "ERC de base funcional" meaning functional CKD, as opposed to "de base orgánica".

Hi Joseph. My bad. Missed it in the heading. It would appear that the "funcional" here is referring to functional kidney failure (anuresis) with no clear cause of kidney dysfunction (because the creatinine levels aren't all that high), so I would translate it as "AKIN 1 AKI on functional CKD in the context of sepsis" . . . or something like that. The unclear diagnoses are the hardest ones to articulate.

There is only one place where "funcional" appears in the entire report and that's in this sentence that I'm inquiring about. The patient was hospitalized for a respiratory infection and has a whole catalog of other diagnoses; this sentence is just one in a list of 5 problems listed under 'otros diagnósticos' (FA paroxistica, deficit de acido folico, hipovitaminosis D, adenocarcinoma de prostata).

I think "de base funcional" is a single phrase and means that the patient's CKD (or CRD if you prefer) was functional (non-organic) rather than organic (pathological). "De base funcional" and "de base orgánica" seem to be used regularly like this. It would also fit in with the reference to "ERC de etiologia no filiada": there was no clear organic origin of the CKD; it was a functional disorder with no detectable structural or biochemical changes.

I'm putting this here rather than posting it as an answer because I don't have time to work out the right rendering of the whole phrase and I'm not 100% certain about this.

I figured out "de sepsis" = "due to sepsis". I still don't know where to put the "funcional".

One slight step forward in this: I think "de base" goes with ERC ... "ERC de base" = underlying chronic kidney disease. I still don't know where to place "funcional" and "de sepsis" ("en el seno" is not a problem).
