Der Vordergrunddienst ist ein Bereitschaftsdienst, bei dem der Arzt im KH anwesend sein muss. Der ganze Dienst ist normalerweise bezahlt, da der Arbeitgeber vorschreibt, wann/wie/wo. Ich würde das als "standby duty" bezeichnen. Das Gegenteil wäre die Rufbereitschaft (Hintergrunddienst), bei der sich der Arzt aufhalten kann, wo er möchte, aber für den Notfall erreichbar sein muss ("on call").

Standby Duty: Whenever an employee is placed on Standby Duty, the employee shall be available at a predetermined location to respond to emergency calls and, when necessary, return immediately to work.

The basic rule is that an employee who is on standby duty must be present for a period of time specified by the employer at a location specified by the employer (aka at the hospital), and must start work immediately when called upon to do so.

(In contrast, an employee on on-call duty must ensure constant availability for a period of time specified by the employer (outside his regular working hours) but may stay at a place of his or her choice from which he or she must take action as required.)
