While I agree with Merab's translation of "durable power of attorney", but I don't think there's a missing comma here; rather, I believe "durable power of attorney" modifies "affidavit".

The above document is an affidavit and indemnification of a power of attorney, wherein the person making the affidavit swears they hold this power and promises to indemnify the party accepting the instructions received under that power of attorney, otherwise the attorney-in-fact must indemnify and hold harmless the accepting party.

  -> Thanks, Alex. I'm not sure about using such a specific term as "indemnidad", the RAE entry you mention says that it relates specifically to German criminal law. Interesting, all the same; I'd never seen that.
