While I agree with Merab's translation of "durable power of attorney", but I don't think there's a missing comma here; rather, I believe "durable power of attorney" modifies "affidavit".

  -> Thanks, Alex. I'm not sure about using such a specific term as "indemnidad", the RAE entry you mention says that it relates specifically to German criminal law. Interesting, all the same; I'd never seen that.
