Source text - English INTRODUCTION
In April 2011, the International Commission on Radiological Protection (ICRP) released a new statement that drastically reduced the putative human threshold values for radiation cataractogenesis to 0.5 Gy from previous values of 2–8 Gy. At the same time, the recommended occupational lens exposure limit was lowered from 150 mSv/year to an average of 20 mSv/year over 5 years, with no single year exceeding 50 mSv. These new recommendations have significant implications for human ocular health risks following occupational, accidental, or terrorist exposures, and highlight a pressing need to better estimate risk of radiation-induced lens pathology. These new recommendations are also likely to affect radiotherapy and diagnostic protocols, as well as hypotheses regarding radiation cataract pathomechanisms.
Until very recently, ocular exposure guidelines were based on the assumption that radiation cataract is a deterministic event requiring threshold doses generally greater than 2 Gy. This view was, in part, based on older studies which generally had short follow up periods, failed to take into account increasing latency as dose decreased, had relatively few subjects with doses below a few Gy, and were not designed to detect early lens changes. Newer findings in populations exposed to far lower doses of radiation, including those undergoing computed tomography scans, radiotherapy, the astronaut pool, atomic bomb survivors, residents of contaminated buildings, Chernobyl accident victims or clean-up, radiological technologists, interventional radiologists, and interventional cardiologists, revealed dose-related lens opacification at significantly lower doses. Experimental animal studies, many from the author’s laboratory, support these conclusions and suggest a role for genotoxicity in the development of radiation cataract. These observations led ICRP to issue the aforementioned April 2011 statement as well as a new draft report on non-cancer effects, including radiation cataract, arising from low-dose irradiation.
THE LENS OF THE EYE AND CATARACT
The lens is an avascular tissue that receives nourishment from its surrounding aqueous and vitreous fluids. Its anatomy is unique, with a single epithelial cell layer on the anterior, corneal-facing surface that contains the progenitors of the underlying lens fibre cells. The lens is completely encased by a basement membrane, termed the ‘lens capsule’. Throughout life, epithelial cells located at the periphery of the lens, in the germinative zone, divide and differentiate into mature lens fibre cells. These terminally differentiated cells do not contain nuclei or mitochondria, and are dependent on the overlying epithelial cell layer for nutrient transport, energy production, and protection from insulting agents. Division slows during puberty, yet the lens continues to grow throughout life, eventually tripling in weight. Damage in the epithelial cell layer is believed to be the initiating event that causes this form of cataract.
The only real pathology of the lens is its opacification, termed ‘cataract’. There are three predominant forms of cataract depending on their anatomical location in the lens: cortical, involving the outer shell and containing more recently formed lens fibre cells; nuclear, found in the oldest, central lens fibre cells; and posterior subcapsular (PSC), developing from aberrantly differentiating epithelial cells and resulting in opacity at the posterior pole. In this regard, despite the presence of an apparently unremarkable lens epithelium and resultant potential for DNA-damaging mutagenic events, it is curious that natural lens tumours have never been reported in the vertebrate eye. It may be that cataract, particularly PSC cataract arising from aberrant differentiation of epithelial cells, may represent lens-specific, cancer-like pathology in the lens resulting from genotoxic mutagenic events that would be carcinogenic in other tissues.

הועדה הבינ"ל להגנה רדיולוגית (ICRP - International Commission on Radiological Protection) פרסמה באפריל 2011 הצהרה חדשה אשר הפחיתה דרסטית את ערכי הסף המשוערים לקרינה הגורמת לירוד (קטרקט) בבני אדם מ 2-8 גריי ל 0.5 גריי. בנוסף, הופחתו ערכי החשיפה התעסוקתית לעדשת העין מ 150 מילי זיוורט לשנה ל 20 מילי זיוורט לשנה בממוצע על פני 5 שנים, כאשר בשום שנה אין לחצות את רף 50 המילי זיוורט. להמלצות חדשות אלו השלכות משמעותיות בכל הנוגע לסיכונים לבריאות העין בעקבות חשיפות תעסוקתיות, תאונות, או מעשי טרור, ושמות דגש על הצורך הבוער להעריך טוב יותר את הסיכון לפתולוגיות עדשתיות עקב קרינה בבני אדם. סביר כי המלצות חדשות אלו ישפיעו על פרוטוקולי רדיותרפיה ואבחנה, כמו גם על ההיפותזות הדנות במנגנונים הפתולוגיים להיווצרות ירוד תלוי קרינה.
עד לאחרונה, הנחיות חשיפה עיניות התבססו על הסברה כי ירוד תלוי קרינה הוא אירוע דטרמיניסטי שבדך כלל מצריך מינוני קרינה הגדולים מ 2 גריי על מנת שיתרחש. תפיסה זו התבססה בחלקה על מחקרי עבר, שלרוב היו בעלי תקופת מעקב קצרה שלא לקחה בחשבון שמינוני קרינה נמוכים מובילים לתקופת חביון ארוכה יותר, כללו מעט נבדקים עם מינוני קרינה הנמוכים ממספר גריי, ובנוסף לא תוכננו לגלות שינויי עדשה מוקדמים. ממצאים חדשים באוכלוסיות אשר נחשפו למינוני קרינה נמוכים בהרבה מאלו שנבדקו בעבר, כגון נבדקי טומוגרפיה ממוחשבת (CT – Computed Tomography), מטופלי רדיותרפיה, אסטרונאוטים, שורדי פצצות אטום, דיירי בניינים נגועי קרינה, קורבנות ועובדי הניקוי של אזור האסון בצ'רנוביל, טכנאי רדיולוגיה, רדיולוגים התערבותיים וקרדיולוגים התערבותיים, גילו עכירויות עדשה הקשורות גם במינוני קרינה נמוכים אלו. מחקרי ניסוי בבעלי חיים, רבים מהם ממעבדת המחבר, תומכים גם הם בכך ומציעים תפקיד לעקה גנוטוקסית בהתפתחות ירוד תלוי קרינה. תצפיות אלו הובילו את ה ICRP לפרסם באפריל 2011 את ההצהרה הנזכרת לעיל, כמו גם טיוטת דו"ח חדש על השפעותיה הלא ממאירות של קרינה במינונים נמוכים, בניהן ירוד תלוי קרינה.
עדשת העין וירוד
העדשה היא רקמה א-וסקולרית שמקבלת הזנה מנוזלי הגוף הריסני והזגוגי. האנטומיה שלה ייחודית וכוללת שכבה בודדת של תאי אפיתל בצידה הקדמי, אשר פונה לקרנית. שכבה זו מכילה את התאים הפרוגניטרים של תאי סיב העדשה שמתחת. העדשה עטופה לחלוטין בממברנת בסיס, הקרויה 'קופסית העדשה'. לאורך החיים, תאי אפיתל הממוקמים בפריפריית העדשה, באזור הנבט, מתחלקים ומתמיינים לתאי סיב עדשה בוגרים. תאים אלו ממוינים לחלוטין, אינם מכילים גרעינים או מיטוכונדריות, ותלויים בשכבת האפיתל שמעליהם להעברת חומרי הזנה, ייצור אנרגיה והגנה מפני גורמים מזיקים. בעוד שחלוקתם מאטה בזמן ההתבגרות, העדשה ממשיכה לגדול לאורך כל החיים, עד לכדי שילוש משקלה. מאמינים כי נזק לשכבת האפיתל הוא הגורם שמחל את תהליך היווצרות סוג זה של ירוד.
