Source text - English 1. INTRODUCTION
1.1 Background
Two medicinal products containing the same active substance are considered bioequivalent if they are
pharmaceutically equivalent or pharmaceutical alternatives and their bioavailabilities (rate and extent)
after administration in the same molar dose lie within acceptable predefined limits. These limits are set
to ensure comparable in vivo performance, i.e. similarity in terms of safety and efficacy.
In bioequivalence studies, the plasma concentration time curve is generally used to assess the rate and
extent of absorption. Selected pharmacokinetic parameters and preset acceptance limits allow the final
decision on bioequivalence of the tested products. AUC, the area under the concentration time curve,
reflects the extent of exposure. Cmax, the maximum plasma concentration or peak exposure, and the
time to maximum plasma concentration, tmax, are parameters that are influenced by absorption rate.
It is the objective of this guideline to specify the requirements for the design, conduct, and evaluation
of bioequivalence studies. The possibility of using in vitro instead of in vivo studies is also addressed.
1.2 Generic medicinal products
In applications for generic medicinal products according to Directive 2001/83/EC, Article 10(1), the
concept of bioequivalence is fundamental. The purpose of establishing bioequivalence is to
demonstrate equivalence in biopharmaceutics quality between the generic medicinal product and a
reference medicinal product in order to allow bridging of preclinical tests and of clinical trials
associated with the reference medicinal product. The current definition for generic medicinal products
is found in Directive 2001/83/EC, Article 10(2)(b), which states that a generic medicinal product is a
product which has the same qualitative and quantitative composition in active substances and the same
pharmaceutical form as the reference medicinal product, and whose bioequivalence with the reference
medicinal product has been demonstrated by appropriate bioavailability studies. The different salts,
esters, ethers, isomers, mixtures of isomers, complexes or derivatives of an active substance are
considered to be the same active substance, unless they differ significantly in properties with regard to
safety and/or efficacy. Furthermore, the various immediate-release oral pharmaceutical forms shall be
considered to be one and the same pharmaceutical form.
1.3 Other types of application
Other types of applications may also require demonstration of bioequivalence, including variations,
fixed combinations, extensions and hybrid applications.
The recommendations on design and conduct given for bioequivalence studies in this guideline may
also be applied to comparative bioavailability studies evaluating different formulations used during the
development of a new medicinal product containing a new chemical entity and to comparative
bioavailability studies included in extension or hybrid applications that are not based exclusively on
bioequivalence data.

1.1 Antecedentes
Dos productos medicinales que contienen la misma sustancia activa se consideran bioequivalentes si son farmacéuticamente equivalentes o alternativos farmacéuticos, y sus biodisponibilidades (tasa y extensión) después de la administración de la misma dosis molar caen dentro de límites predefinidos aceptables. Estos límites se definen para asegurar un comportamiento in vivo comparable, es decir, similitud en términos de eficacia y seguridad.
En los estudios de bioequivalencia, la curva de tiempo de la concentración plasmática generalmente se utiliza para evaluar la tasa y la extensión de la absorción. Los parámetros farmacocinéticos seleccionados y los límites de aceptación predefinidos permiten la decisión final acerca de la bioequivalencia de los productos analizados. El área bajo la curva de tiempo de la concentración (AUC, por sus siglas en inglés), refleja la exposición. Cmax, la concentración plasmática máxima o exposición máxima; y el tiempo hasta la máxima concentración plasmática, tmax, son parámetros que se ven influenciados por la tasa de absorción.
Es el objetivo de esta guía el especificar los requerimientos para el diseño, conducción y evaluación de los estudios de bioequivalencia. También se aborda la posibilidad de utilizar estudios in vitro en lugar de estudios in vivo.
1.2 Productos medicinales genéricos
En las solicitudes para productos medicinales genéricos de acuerdo a la Directiva 2001/83/EC, Artículo 10(1), el concepto de bioequivalencia es fundamental. El propósito de establecer la bioequivalencia es el demostrar la equivalencia de la calidad biofarmacéutica entre el producto medicinal genérico y un producto medicinal de referencia para permitir el puenteo de las pruebas preclínicas y de los ensayos clínicos asociados con el producto medicinal de referencia. La definición actual para productos medicinales genéricos se encuentra en la Directiva 2001/83/EC, Artículo 10(2)(b), la cual establece que un producto medicinal genérico es un producto que tiene la misma composición cualitativa y cuantitativa de las sustancias activas y la misma forma farmacéutica que el producto medicinal de referencia, y cuya bioequivalencia con el producto medicinal de referencia ha sido demostrada por medio de estudios de biodisponibilidad apropiados. Las diferentes sales, ésteres, éteres, isómeros, mezclas de isómeros, complejos o derivados de una sustancia activa son considerados como la misma sustancia activa, a menos que difieran significativamente en las propiedades referentes a seguridad y/o eficacia. Además, las diferentes formas farmacéuticas orales de liberación inmediata deben considerarse como una y la misma forma farmacéutica.
1.3 Otros tipos de aplicación
Otros tipos de solicitudes también pueden requerir demostrar la bioequivalencia, incluyendo las variaciones, combinaciones fijas, extensiones y solicitudes de híbridos.
Las recomendaciones acerca del diseño y conducción dadas en esta guía para los estudios de bioequivalencia también pueden aplicarse a estudios de biodisponibilidad comparativa que evalúen diferentes formulaciones utilizadas durante el desarrollo de un nuevo producto medicinal que contenga una nueva entidad química, y para comparar estudios de biodisponibilidad incluidos en solicitudes de extensión o de híbridos que no estén basadas exclusivamente en datos de bioequivalencia.

4. Procedimiento: Reactivos especiales.
Solución de referencia de Nitrato de Plomo: Disolver 159.8 mg de Nitrato de Plomo en 100 mL de agua a la cual se le ha agregado 1 mL de HN03, luego diluir con agua a 1000 mL. Solución estándar de Plomo: El día de su uso, diluir 10 mL de solución de referencia de Nitrato de Plomo con agua a 100 mL.
Buffer de acetato pH 3.5: Disolver 25.0 g de acetato de amonio en 25 mL de agua, y 38 mL de ácido clorhídrico 6N. Ajustar, si es necesario, el pH de la solución a 3.5, con hidróxido de amonio 6N o ácido clorhídrico 6N, diluir con agua a 100 mL, y mezclar.
Tioacetamida SR: Disolver 4 g de tioacetamida en 100 mL de agua.
Glicerina Base SR: A 200g de glicerina, agregar agua hasta hacer un peso de 235g. Agregar 140 mL de hidróxido de sodio 1 N Y 50 mL de agua.
Tioacetamida-glicerina Base SR: Mezclar 0.2 mL de tioacetamida SR con 1 mL de glicerina SR, y calentar en un baño de agua en ebullición durante 20 segundos. Usar esta mezcla inmediatamente.
Preparación de la muestra:
• Pesar 2 g de la muestra en un crisol, adicionar suficiente Acido Sulfúrico SR para humedecer la muestra y con cuidado incinerar a baja temperatura hasta que se carbonice totalmente.
• Durante la carbonización el crisol puede cubrirse parcialmente.
• Agregar a la muestra carbonizada 2 mL de Acido Nítrico y 5 gotas de Acido Sulfúrico SR y calentar cuidadosamente hasta que no haya más desprendimiento de vapores blancos. Someter a ignición, de preferencia en una mufla entre 500 - 600°C hasta que el carbón se haya quemado completamente.
• Enfriar, agregar 4 mL de solución de Acido Clorhídrico 6N SV, tapar y digerir en Baño maría durante 15 minutos, destapar y evaporar lentamente hasta sequedad. Humedecer el residuo con 1 gota de Acido Clorhídrico SR, agregar 10 mL de agua caliente y digerir 2 minutos. Agregar gota a gota solución de Hidróxido de Amonio 6 N SV hasta que la solución esté alcalina al papel indicador de pH (Tiras reactivas).
• Diluir con agua a 25 mL y ajustar el pH entre 3.0 - 4.0 con Acido Acético 1 N SV. Filtrar si es necesario, enjuagar el crisol y el filtro con 10 mL de agua, combinar el filtrado y el lavado, diluir con agua hasta 40 mL y mezclar en un tubo de Nessler.
• Preparación de la solución de Referencia: En un tubo Nessler de 50 mL, tomar una alícuota de 2 mL de solución estándar de Plomo (20 mg), y diluir con agua a 25 mL. Ajustar el pH entre 3.0 y 4.0, con Acido Acético 1 N o Hidróxido de Amonio 6N, diluir con agua y mezclar.
• A cada uno de los tubos que contienen la preparación de referencia y de la muestra respectivamente, adicionar 2 mL de Buffer acetato pH 3.5 Y 1.2 mL de tioacetamida-glicerina base SR. Diluir con agua a 50 mL y dejar reposar 2 minutos.
• Comparar los tubos de arriba hacia abajo sobre una superficie blanca.
• El color de la muestra no debe ser más oscuro que el color de la solución de referencia.

2. Reagents:
• Acetic Acid 1N TS.
• Sulfuric Acid RS.
• Nitric Acid RS.
• Chlorhydric Acid 6N TS.
• Ammonium Hydroxide 6N TS.
• Thioacetamide-glycerin base RS.
• Lead nitrate.
• Acetate buffer pH 3.5.
• Purified water.
• Chlorhydric acid RS.

• Muffle furnace.

4. Procedure: Special reagents.
Lead Nitrate Reference solution: Dissolve 159.8 mg of Lead Nitrate in 100 mL of water to which 1 mL of HNO3 have been added, then dilute with water to 1000 mL.
Standard Lead solution: On the day of its use, dilute 10 mL of Lead Nitrate reference solution with water to 100 mL.
Acetate buffer pH 3.5: Dissolve 25.0 g of ammonium acetate in 25 mL of water, and 38 mL of chlorhydric acid 6N. Adjust, if necessary, the pH of the solution at 3.5, with ammonium hydroxide 6N or chlorhydric acid 6N, dilute with water to 100 mL, and mix.
Thioacetamide RS: Dissolve 4 g of thioacetamide in 100 mL of water.
Glycerin base RS: To 200 g of glycerin, add water until achieving a weight of 235 g. Add 140 mL of sodium hydroxide 1N and 50 mL of water.
Thioacetamide-glycerin base RS: Mix 0.2 mL of thioacetamide RS with 1 mL of glycerin RS, and heat in a boiling water bath for 20 seconds. Use this mixture immediately.
Sample preparation:
• Weigh 2 g of the sample in a crucible, add enough Sulfuric Acid RS to moisten the sample and carefully incinerate at low temperature until it is completely carbonized.
• During the carbonization the crucible may be partially covered.
• Add to the carbonized sample 2 mL of Nitric Acid and 5 drops of Sulfuric Acid RS and carefully heat until there is no more emission of white vapors. Subject to ignition, preferably into a muffle furnace between 500 – 600ºC until the carbon has completely charred.
• Cool down, add 4 mL of Chlorhydric Acid TS 6N solution, cover and digest in a water bath for 15 minutes, uncover and slowly evaporate to dryness. Moisten the residue with 1 drop of Chlorhydric Acid RS, add 10 mL of hot water and digest for 2 minutes. Add Ammonium Hydroxide TS 6 N solution drop by drop until the solution is alkaline to the pH indicator paper (Reaction strips).
• Dilute with water to 25 mL and adjust the pH between 3.0 – 4.0, with Acetic Acid TS 1N. Filter if necessary, rinse the crucible and the filter with 10 mL of water, combine the filtrate and the washed, dilute with water to 40 mL and mix in a Nessler tube.
• Preparation of the Reference solution: In a Nessler tube of 50 mL, take an aliquot of 2 mL of standard Lead solution (20 mg), and dilute with water to 25 mL. Adjust the pH between 3.0 and 4.0, with Acetic Acid 1N or Ammonium Hydroxide 6N, dilute with water and mix.
• To each one of the tubes containing the reference preparation and the sample, respectively, add 2 mL of acetate Buffer pH 3.5 and 1.2 mL of thioacetamide-glycerin base RS. Dilute with water to 50 mL and leave to settle for 2 minutes.
• Compare the tubes from top to bottom over a white surface.
• The color of the sample must not be darker than the color of the reference solution.
