Source text - English The indications for cranioplasty after decompressive craniectomy are cosmetic repair and, mainly, restoration of cerebral protection. Although neurological improvement after cranioplasty is repeatedly noted, the reasons for this still remain unclear. Few observations concerning the impact of CSF hydrodynamic and/or atmospheric pressure were published during the last decades. Relevant data concerning the cerebrovascular reserve capacity and cerebral glucose metabolism before and after cranioplasty have been lacking until now. To gain further insight, the present study was undertaken to investigate the impact of cranioplasty on indices of cerebral blood flow regulation and metabolism. Thirteen patients in whom extensive craniectomies had been performed underwent a meticulous study of blood flow velocities in the middle cerebral artery (MCA) and extracranial internal carotid artery (ICA).

Iako se neurološko poboljšanje nakon kranioplastike opetovano uočava, razlozi za to još uvijek ostaju nejasni. Tijekom posljednjih desetljeća objavljeno je nekoliko studija o utjecaju CSL hidrodinamičkog i / ili atmosferskog pritiska. Relevantni podaci o zalihama cerebrovaskularnih sadržina i cerebralnog glukoznog metabolizma prije i poslije kranioplastike do sada su bili malo poznati. Naša djevojčica je bila u dobi od 17 mj. i teška 11 kg. Tri dana je zbog febriliteta liječena kod kuće paracetamolom, navodno u terapijskim dozama, i antibiotikom. Zbog febriliteta je hospitalizirana u vanjskoj ustanovi gdje je nastavljeno antibiotsko liječenje zbog dokazane urinarne infekcije. Ta tri dana bila je i febrilna, a nemamo podatka o antipiretskim mjerama. Treći dan boravka dolazi do pogoršanja općeg stanja s povraćanjem i adinamijom. Laboratorijski je nađena metabolička acidoza. Sljedeći dan dolazi do poboljšanja stanja. Dan kasnije nastupa somnolencija koja se brzo razvija u sopor, ikterus praćen hepatomegalijom i bradikardija. AST je bila 6200 U/L. Pod sumnjom na Reyev sindrom djevojčica je premještena u naš Zavod.
Zbog kliničke slike jetrene insuficijencije nejasna uzroka nakon dolaska je učinjena toksikološka analiza urina u kojoj su nađene velike količine paracetamola (205,88 μmol/L). Koncentracija paracetamola u krvi bila je tada 9 μmol/L. Na temelju anamneze o uzimanju paracetamola, kliničke slike i laboratorijskih nalaza postavljena je dijagnoza otrovanja paracetamolom.

Our little girl was 17 months old and 11 kilos weight. She was treated for febrility at home for three days with paracetamol, apparently in therapeutic doses, and antibiotics. Because of the febrility, was hospitalized at the outside institution where antibiotic treatment was continued due to the diagnosed urinary infection. During those three days she was febrile too, while we had no data on antipyretic measures. On the third day of her stay, deterioration of general condition with vomiting and adynamia occurred. Laboratory testing has shown a metabolic acidosis. Next day her condition had improved. A day later somnolence appeared, which quickly developed into a sopor, jaundice accompanied by hepatomegaly and bradycardia. AST was 6200 U / L. Under the suspicion of Reye's syndrome, little girl was transferred to our Department. Due to the clinical feature of hepatic insufficiency of unclear cause, upon arrival she underwent toxicological analysis of urine samples in which a large quantities of acetaminophen were found (205.88 µmol / L). The concentration of paracetamol in blood was then 9 µmol. On the basis of history taking paracetamol, clinical feature and laboratory findings, she was diagnosed with paracetamol poisoning.

2. QUALITATIVE AND QUANTITATIVE COMPOSITION

Fentanyl 50 mg/ml

3. PHARMACEUTICAL FORM
Cutaneous solution.
Clear, colourless to light yellow solution.
4. CLINICAL PARTICULARS
4.1 Target species
Dogs
4.2 Indications for use, specifying the target species
For the control of post-operative pain associated with major orthopaedic and soft tissue surgery in dogs.
4.3 Contraindications
Do not administer to skin that does not have an intact stratum corneum due to injury or disease.
Do not administer to areas other than the dorsal scapular region.
Do not use in dogs with cardiac failure, hypotension, hypovolaemia, respiratory depression, hypertension, a history of epilepsy, non age related corneal pathology or those who have or are suspected of having a paralytic ileus.
Do not use in case of hypersensitivity to the active substance or to any of the excipients.
Do not give a second dose of the veterinary medicinal product. Accumulation of fentanyl following repeated administration could result in severe adverse reactions, including death. Do not give more than the recommended dose of the veterinary medicinal product.
Do not use the veterinary medicinal product in lactating or pregnant bitches or breeding animals (see section 4.7).
4.4 Special warnings
Only use the syringes provided. The use of syringes not supplied with this veterinary medicinal product, or storage of this veterinary medicinal product in a syringe may lead to dosing inaccuracy. Do not re-use syringes or applicator tips.
The use of the veterinary medicinal product is intended as a single application administered 2 to 4 hours prior to surgery to provide analgesia for at least 4 days. Should subsequent surgery be intended in a dog previously treated with the veterinary medicinal product, a minimum of a 7 day dosing interval must be observed before administering another dose.

4.4 Posebna upozorenja
