Source text - German Herr A. verunfallte in der Nacht vom 08.12.98 auf den 09.12.98 um 22.05 Uhr. Er war in eine Glasscherbe gefallen.
Bei dem Unfall war es zu einer Durchtrennung der beiden Beugesehnen des IV. Fingers der linken Hand sowie des gemeinsamen Fingernerven 8 und 9 gekommen. Noch am Unfalltag erfolgte die Revision in Blutleere. Hierbei fand sich eine glatte Durchtrennung der oberfl. und tiefen Beugesehne des IV. Fingers in Höhe der Hohlhandmitte. Weiterhin zeigte sich eine Durchtrennung des gemeinsamen Fingernerven N 8/9.
Nach Erweiterung der Schnittführung konnte hier lediglich der proximale Stumpf der tiefen Beugesehne identifiziert werden. Auf eine Revision im Unterarmbereich wurde verzichtet. Stattdessen wurde hier lediglich die tiefe Beugesehne modifiziert nach Kleinert versorgt. Gleichzeitig erfolgte die mikrochirurgische Rekonstruktion der durchtrennten Fingernerven mit 10.0 Einzelknopfnähten. Postoperativ erhielt der Patient einen Kleinert-Gips. Am nächsten Tag wurde mit der Kleinert'schen Übungsbehandlung begonnen. Am 15.12.98 wurde in orthograde Stellung umgegipst und der Pat. hieraus weiter beübt.
Wir möchten Sie bitten, die begonnenen Kleinert'sche Übungsbehandlung fortzuführen. Es ist geplant, den Gipsverband für insgesamt 4 Wochen zu belassen. Anschließend sollte mit aktiven Bewegungsübungen für 2 Wochen begonnen werden. Nach insgesamt 6 Wochen ist auch eine passive Übungsbehandlung vorgesehen
Translation - English Mr. A. suffered an accident during the night between December 8th and 9th, 1998 at 10:05 pm. He had fallen into a shard of glass.
The accident resulted in a transection of both flexor tendons of the fourth finger of the left hand, as well as the combined digital nerves 8 and 9 . On the day of the accident I performed a bloodless examination. I found a clean transection of the superficial and deep flexor tendons of the fourth finger in the middle of palm. Further, a transection of the combined digital nerves 8 and 9 was revealed. After widening the incision only the proximal stump of the deep flexor tendon could be identified. I rejected the idea of a repair procedure in the forearm. Instead, I simply treated the deep flexor tendon using the Kleinert technique . A microsurgical reconstruction of the severed digital nerves took place simultaneously, using 10 simple interrupted sutures. The patient received a Kleinert plaster splint post-operatively. The next day the patient began the Kleinert motion therapy. On December 12, 1998, I recast the hand in orthograde position and treatment was continued.
I request that the Kleinert motion therapy be continued. The plan is for the splint to be left on for a total of four (4) weeks. Subsequent to this, two (2) weeks of active motion therapy should follow. Passive motion therapy is also indicated after a total of six (6) weeks.
